Community Bridges is currently seeking an Overnight Peer Support Specialist in Mesa, Avondale, Casa Grande, and Phoenix locations.

The Peer Support Specialist is a member of the clinical team dedicated to clients. Coordinates services based on daily staffing's with other members of the clinical team, provides experience, strength, and hope to individuals, their families, and significant others by sharing their recovery experience and knowledge of the behavioral health system and available community resources to promote recovery and self-improvement.

ESSENTIAL JOB DUTIES OR RESPONSIBILITIES

  • Monitors daily activities and coordinates care with supportive members and providers.
  • Provides in field outreach and checks to ensure member is meeting desired treatment goals.
  • Ensures required face to face interaction with each assigned client is provided at least weekly and documented accordingly.
  • Facilitates peer led groups.
  • Coordinates with family members and team on service delivery methods and options.
  • Participates in daily staffing with other team members.
  • Coordinates with Program Assistant to ensure that appointments are being met and that medications are available.
  • Provides in-field crisis and other related supportive services
  • Serves as an advisor/peer to other ACT Team members to make recommendations and carry out services.
  • Provides services in the field, at the member’s residence or wherever the member may be located at the time services are needed.
  • Works with member and family/natural supports to provide education on needed services as well as advocate for any additional services identified.
  • Works with members to develop a role for themselves outside of their mental illness, while focusing on expected outcomes of increased employment, Meaningful Community Activity (MCA), independent living status, and social network, as well as decreased substance use/abuse.
  • Teaches skill building activities to the member.
  • Performs case management duties as defined in each member’s individualized treatment plan.
  • Is an active participant in each member’s individualized at-risk crisis plan.
  • Documents interactions with each member daily prior to the end of his or her scheduled shift.
  • Participates in daily staffing and provides insight and updates on the member’s progress as well as recommendations for additional services that will help the member be successful.
  • Assists in move-in activities.
  • Provides transportation to appointments and/or accompanies the member using public transportation.
  • Provides outreach and engagement activities daily in addition to after hour response while on-call or when member requests assistance.
  • Participates in weekly team afterhours on-call rotation.
  • Engage members during the screening process to build rapport and understand benefits of ACT level of care.
  • Other duties as assigned by supervisory and/or administrative personnel.

SUPERVISORY RESPONSIBILITIES

This position has no supervisory responsibilities.

MINIMUM EDUCATION & EXPERIENCE REQUIRED

  • Highschool diploma or GED required.
  • Associate Degree or higher preferred.
  • Must be in recovery from Alcohol or Drugs and/or GMH for equal to or greater than 6 months.
  • 1-3 years of experience working in the Behavioral Health and/or Substance Abuse field preferred.
  • Or equivalent combination of education and experience.

LICENSES, CERTIFICATIONS, CREDENTIALS, REGISTRATIONS

  • Current AZ Driver’s License
  • Clean 39-month Motor Vehicle Record
  • Certified Peer Support Specialist certification
