OUTREACH WORKER FOR LATIN SENIORS IN WALTHAM (Part-Time)

Statement of Duties:

In collaboration with the Waltham Council on Aging and Waltham Connections for Healthy Aging, the Outreach Worker develops and implements programs and services that meet the social, emotional, economic and health needs of Waltham’s Latino senior population. Connections is a senior-led organization promoting healthy aging that works closely with the Council on Aging. Connections pays the salary of the Outreach Worker through private grant funding.

Supervision:

The Worker reports to the Council on Aging Director. Performs varied duties requiring considerable initiativeand independent judgment in assessing service programs, working with volunteers, and addressing issues of the city’s Latin senior population. Works in tandem with the other Council on Aging staff.

Job Environment:

Work is generally performed at the Waltham Senior Center in typical office conditions. Work is subject to interruptions by telephone calls and visitors. Work at times involves dealing with sensitive and confidential situations, such as physical and/or emotional needs of elders. Work may include visits to churches, senior housing facilities, and local businesses as needed. Operation of an automobile, computer, telephone, and other standard office equipment is regular. The Outreach Worker has consistent contact with the public. Contacts are in person, by phone (outreach line at the Senior Center), and in writing.

Essential Functions:

  • Provides interpretation and translation services between English and Spanish as needed.
  • Conducts a basic intake and evaluation process for seniors to determine needs and establish most appropriate referral or services options.
  • Provides referrals to appropriate resources and agencies to address unique needs of seniors and their families.
  • Advocates for seniors for provision of resources such as fuel assistance, including coordinating and completing required paperwork.
  • Maintains client records through data entry and prepare weekly timesheet reports.
  • Performs varied outreach duties to inform seniors of programs and services and available resources.
  • Conducts in-home visits to homebound or frail elders as needed to assess needs and follow-up to insure adequacy of assistance.
  • Regularly consults with family members and community agencies.
  • Plans and attends outings with seniors.
  • Alerts Council on Aging Director to potential issues. Works collaboratively with the town’s Social Service Manager with elders who are in need of clinical services.
  • Implements initiatives and/or programs at the direction of the Council on Aging. Identifies new and existing community resources, provides updates and information to seniors and caretakers. Encourages participation at the senior center to promote community involvement.
  • On behalf of Waltham Connections and the Council on Aging, participates in local, regional and state meetings as necessary to expand and improve services to older adults. Networks with health care providers, social service organizations, long-term care providers and other groups in the elder service network.
  • Fundraises through local businesses for events and activities.
  • In collaboration with Waltham Connections, conducts research to collect information on various resources based on client needs.
  • Performs additional work as required.

Recommended Qualifications:

Associate’s or Bachelor’s Degree in human services, social work, gerontology, or a related field is preferred. Minimum two (2) years of experience in social service, preferably working with an elderly population is desired. Prior municipal experience and experience in technology applications are helpful.

Special Requirements:

Massachusetts Driver’s license required. Certification in CPR, first aid, and AED desirable. Flexibility, patience, and ability to establish boundaries.

Conditions:

  • Part-time, temporary grant-funded position, no benefits.
  • Hours: 16 hours per week.
  • Weekly work schedule to be determined with supervisor.
