Job Title: Occupational Therapist (OT) - Home Health

Location: San Jose, CA

Type: 13-week renewable contract (monthly income guaranteed)

Job Description:

We are seeking a skilled and experienced Occupational Therapist (OT) to join our team on a 13-week renewable contract basis. In this role, you will provide high-quality occupational therapy services to patients in their homes. You will work closely with the patient's care team to develop and implement treatment plans that help patients achieve their goals and maximize their independence.

Occupational Therapist- Field position
Location: San Jose coverage area
Shift: Days Mon-Fri
CA license in hand
Experience: 1 year of Home Health experience required
Oasis experience preferred
HCHB experience is preferred but will train.
Mileage at IRS standard rate between patients.
COVID Vaccination preferred- Exemptions permitted with weekly testing

Responsibilities:

• Conduct comprehensive assessments of patient's physical, cognitive, and functional abilities
• Develop individualized treatment plans based on patient goals and needs
• Provide hands-on therapy to improve patient's ability to perform daily living activities, such as dressing, grooming, and eating
• Teach patients exercises and activities to improve strength, coordination, and range of motion
• Monitor patients' progress and adjust treatment plans as needed
• Educate patients and their families on how to safely perform activities of daily living and use adaptive equipment
• Maintain accurate and complete patient records in compliance with regulatory requirements

Qualifications:

• Bachelor's or master's degree in Occupational Therapy from an accredited program
• Current and valid California state licensure as an Occupational Therapist
• Minimum of 2 years of experience as an OT, with at least 1 year of experience in home health
• Excellent communication and interpersonal skills
• Ability to work independently and as part of a team
• Current BLS certification
