Using own automobile, drives to patients/clients residence and provides care within the established care plan for the patient/client including bathing, personal hygiene/care and grooming and homemaker duties. Follows instructions from the case RN or the manager in providing care and reports any patient/client concerns to the appropriate person.

There are no assigned regions for this position. The schedule is primarily based on the client/patient need and the staff will be scheduled in all areas island-wide based on operational need.

This position is required to work weekend rotations. When scheduled to work weekends (Saturday and Sunday), Hospice Aides will have one (1) scheduled day off prior to the weekend they are scheduled to work and one (1) scheduled day off the following week of the weekend worked to maintain the 40-hour work week.

Essential Functions of the Job:

Provides care within the established care plan for the patient/client, demonstrating sensitivity to the specific client’s needs.

  • Bathes and shampoos patient/client in bed, tub or shower.
  • Does peri-care.
  • Changes bed linens, as needed, ensuring immediate environment is safe and clean.
  • Provides for measures which assist in the safety of the client/patient, such as side rails, bumper pads, etc.
  • Performs related personal care tasks, as instructed or assigned, including, but not limited to, hygiene, shaving, brushing/cleaning of teeth and/or wiping out mouth, brushing of hair.
  • Performs simple non-sterile dressing changes and foley catheter care.
  • Assists patient in dressing and applying assistive devices, when necessary
  • Assists patient with ambulation and may give ROM exercises.
  • May give selected enemas, under the direction of a RN
  • Carries out isolation techniques, when appropriate.
  • Does weekly checks of DME for cleanliness and safety.
  • Delivers and picks up supplies/equipment, as needed.
  • Provides homemaker duties, when needed, including but not limited to, light housekeeping, preparing meals and laundry.
  • Does post-mortem care.
  • Any other related patient/client tasks as instructed/assigned.

Collects specimens, as necessary (i.e. sputum, urine and feces). May provide instruction and/or demonstrate to family/care giver how to assist pt with ambulation, transfer pt to/from bed, to wheelchair, to commode, etc. and how to turn or change pts position in bed. Adheres to proper safety measures and infection control procedures/practices with regards to patient and employee safety. Maintains patient/client privacy and confidentiality. Maintains a professional and courteous demeanor with patients/clients/caregivers/families at all times. Maintain cleanliness in all areas that care was provided to client/patient. Attends meetings and training sessions, as required. Prepares and participates in regulatory/accrediting (i.e. DOH/JCAHO).

Travels to home or residence using own vehicle, with proper registration and insurance and valid Hawaii driver’s license

  • Provides manager with copies of current valid Hawaii Driver’s license, current insurance card and current vehicle registration and updates copies provided, as needed.
  • Completes and submits request for reimbursement of mileage in compliance with the company policy.
  • Promptly reports any vehicle accidents that occur during work time to the manager and any infractions which may affect the ability of the employee to drive for work.

Promptly reports the following to RN (Hospice pts) or Manager (Bathing clients):

  • Any suspicion of neglect/abuse
  • Changes in food/liquid intake
  • Documents and reports all falls or unusual occurrences using an incident report
  • Any needs for social service/bereavement support or conditions relevant to clients social and emotional needs.
  • Any changes in patient’s mental of physical condition.

Performs other related duties as required.

Educational Requirements:

High School Graduate or equivalent. Certificate of completion of a State of Hawaii certified Nurse’s Aide course (100 hours). State of Hawaii Nurse Aide Certification, preferred.

Skills Required:

Good communication skills, with ability to speak and write in English and clearly/effectively communicate. Good customer service and interpersonal skills with ability to be patient, maintain composure and interact with customers with courtesy, tact and discretion. Dexterity in handling equipment and instruments. Ability to safely operate an automobile - must have personal automobile available for business use, valid/current driver’s license and good driving record as evidenced by current traffic abstract. Ability to work independently. Current healthcare provider CPR certification.

Communication and Contacts:

Requires contacts both inside and outside the clients’ homes and St. Francis facilities in order to furnish or obtain information and/or provide patient care. Normal courtesy, tact and discretion required. Must be able to listen/understand what the customer needs are and clearly communicate with the customer. Job requires employees to communicate in English while in public/common areas where conversations can be overheard by others.
