We are bold innovators, dedicated to making a difference and we hire wicked smart people! Our Nurse Practitioners (NP) are responsible for providing health care to patients in their home. This position will Cover the West Valley and see patients in Assisted Living Facilities.
This position offers a great work/life balance with no overnights, weekends, or holidays!

Requirements:
- 2 years of experience with caring for adult & chronically ill patients
- 2 years of experience with home health and/or case management
- Must be willing to travel up to a 50-mile radius from home
- Must be Covid vaccinated
