Details:

  • Monday-Thursday, 7a-5:30pm, Full Time to start ASAC
  • Occasional Saturday, but would be given another day off the week they would work Saturday.
  • Average patients per day: about 17.
  • Duties: Family practice, 1 year to 100+ of age. Urgent care experience, well-child visits, and preventative health.
  • Required Procedures: Pap smear, sutures, minor procedures.
  • Support Staff: Would have their own MA.
  • Reason for coverage: Provider left practice.
  • Setting: Clinic
  • FTE: 1.0 (at least 2 years experience)
  • EMR: Cerner

Credentialing Information:

  • Timeframe: 30-45 days
  • Requirements:
  • Credentialing with other facilities? No
  • Billing enrollment - Is this position providing services in lieu of an existing enrolled provider for a period of 60 days or less? NO

Travel Preferences:

  • Preferred Lodging: Lake Shastina Golf Resort - Must align with Travel Policy/GSA rates

Rates to Submit:

  • Orientation (half the hourly rate, kicks in after 8 hours)
  • Hourly (per hour, no OT)
  • Holiday (1.5x the regualar rate)

AANP/NCCPA Certified - REQUIRED
Active CA License - REQUIRED
Active BLS, DEA - REQUIRED
Full Time Availability - REQUIRED
Clean Malpractice/background - REQUIRED
Must have at least 2 years of experience - REQUIRED,.
COVID VACCINATED - REQUIRED.
