Responsible for triaging incoming and outgoing patient’s calls, assessing social and clinical needs, giving appropriate clinical dispositions and facilitating referrals to community based organizations, other Population Health disciplines and to WVU Medicine primary care and specialty care providers. The Nurse Navigator will educate the patient when appropriate regarding immediate care advice and preventive behaviors. Will be available 24 hours a day, 7 days a week.

MINIMUM QUALIFICATIONS :

EDUCATION, CERTIFICATION, AND/OR LICENSURE:

1. Current West Virginia licensure as a Registered Nurse required upon hire.

Licensure as a Registered Nurse in the following states is required within 6 months of hire: MD, PA, OH, KY, and VA.

2. Obtain certification in Basic Life Support within 30 days of hire date.

EXPERIENCE:

1. Three (3) years of nursing experience.

PREFERRED QUALIFICATIONS :

EDUCATION, CERTIFICATION, AND/OR LICENSURE:

1. Bachelor of Science in Nursing (BSN).

EXPERIENCE:

1. Care coordination experience is preferred.

CORE DUTIES AND RESPONSIBILITIES: The statements described here are intended to describe the general nature of work being performed by people assigned to this position. 1. Professional assessment of caller’s physical and psychosocial needs using established criteria and protocols while performing accurate, concise and timely documentation.

2. Use established software, on-line resources, written materials and nursing background to provide callers with appropriate medical information.

3. Assesses the complexity of care needs and potential/actual issues or gaps in care.

4. Facilitates referrals within internal and external clinics by scheduling and coordinating either a clinic appointment, e-visit or telehealth visit with a provider.

5. Makes post discharge transitions of care phone call to ensure a safe and timely transition from the hospital or ED.

6. Post discharge (Inpatient and ED) calls include monitoring the achievement of clinical outcomes and communicating with inpatients teams, primary and specialty providers, and community resources regarding unanticipated problems.

7. Assists with arranging post-discharge medical and community referrals for patients with health problems requiring further evaluation and/or additional services.

8. Advocates for patients and families within the health care system with community providers and across the continuum of care.

9. Works with Emergency Department physicians to coordinate the care of patients to ensure the appropriate level of care is received.

10. Collaborates with available social services for appropriate resource and financial management which may include, but is not limited to financial assistance coordination/referrals, entitlement program coordination/referrals, patient benefit coordination, assessment for appropriate usage of Health Care Resources/clinical cost efficiency.

11. Supports consumers in selection of best physician and facility to maximize access, quality, and to manage heath care cost.

12. Provides health education and coaches consumers on treatment alternatives to assist them in best decision-making.

13. Identifies the nature and urgency of patient’s health care needs and determine appropriate disposition via telephone.

14. Documents all conversations with patients to maintain a comprehensive medical record.

15. Supports a team environment which allows clear communication, positive support and creative thinking.

Additional Job Description:

Fully remote option available - rotational position is primarily 7a-7p with occasional 7p-7a and rotating weekend coverage
