As a Nurse Care Manager, you will work in the Certified Community Behavioral Health Clinic located in Mercer County NJ. The program serves adults with serious mental illness (SMI), substance use disorders (SUD), co-occurring mental and substance disorders (COD) and youth and adolescents with serious emotional disturbances (SED).

Schedule: Full-time; Monday-Friday 9:00am-5:00pm

Responsibilities:

• Understands target populations and specialized needs; Develops knowledge and skills appropriate to the target population and necessary completion of responsibilities;
• Works with individuals in the community who have co-morbid physical health diagnoses to help coordinate care with primary/specialty providers and behavioral health treatment;
• Secures screening, preventative care, and early intervention for medical problems, including lab work; annual physicals, gynecological, eye, hearing, and dental exams.
• Administers and delivers medications, monitors physical health and works to ensure individuals are receiving necessary care in the community with the goal of reducing utilization of acute services;
• Assists in providing liaison between acute services and person served returning successfully to community-based services;
• Provides education and support for healthy lifestyle and nutritional changes, including reduced dependence on tobacco, alcohol, and other harmful substances;
• Assists consumers in obtaining pharmaceutical coverage and prior authorizations;
• Provide disease management and illness management services, including evidence-based motivational interventions to increase treatment compliance, support lifestyle changes, and manage the co-morbid anxiety/depression that often accompany chronic diseases;
• Provides injections, monitors compliance with medications, reviews lab reports, contacts pharmacies on behalf of prescribers;
• Completes preliminary health screenings and arranges for clients to obtain physicals and other medical care as needed;
• Engages consumers in monitoring health and wellness tracking key health indicators;
• Prepares documentation thoroughly and on a timely basis in the electronic health record;
• Participates in multi-disciplinary team meetings based on agency standards
• Participates in continued education/training sessions, quality assurance/utilization and peer review activities and other program activities as assigned
• Attends individual supervision based on agency standards;
• Makes recommendation to supervisor regarding program policies and procedures;
• Other duties as assigned.

Qualifications:

• Licensed by the State of New Jersey as a professional nurse pursuant to N.J.S.A; Licensed Practical Nurse (LPN) or Registered Nurse (RN); license must be in good standing; must have advanced training
• Experience in provision of behavioral health services; minimally 6 months nursing experience required;
• Valid Driver's License in good standing
