Position Overview: The Case Manager III is a social worker who has obtained a Master in Social Work degree. They will be part of the Anaheim Community Care Response Team responding to 9-1-1 and Anaheim Anytime calls for service for people experiencing homelessness and mental health concerns. Social workers will provide mental health assessments, meet clients where they are, and support them to achieve their goals. City Net’s services are housing-focused; Social Workers work with clients on developing long-term housing plans. Social Workers connect homeless neighbors to appropriate supportive services, including medical health treatment, mental health treatment, counseling, and permanent housing.

Social Workers will be part of community care response teams alongside Drug and Alcohol Counselors, Case Managers, Psych Techs, and other professionals offering care and service coordination in partnership with other nonprofits, County and City entities, law enforcement, behavioral health services, and medical providers to ensure clients receive the help they need. These services are provided in collaboration with our partners with the intention of assisting clients with obtaining and maintaining permanent housing as quickly as possible.

Please note: while this job posting is for an opening we currently have in the City mentioned above, we are responsive to the needs of our clients and funders, and therefore the person in this position may be transferred to neighboring locations as needed.

Main Work Location: Reports daily to the Anaheim office to be dispatched to field work throughout Anaheim.

Work Environment and Expected Hours: This is a regular, full time, hourly position that operates primarily “in the field” (not in an office or from home), with daily travel to and work within local neighborhoods, shelters, and informal encampments as well as, under normal circumstances, meetings in professional office environments, law enforcement offices, community nonprofit and government agency offices, and other public and business spaces throughout the area. This is an essential, client-facing position during any crisis. This position works four days per week, 10 hours each shift.
Shift 1: 10:30 am - 9:00 pm, Wednesday- Saturday

Requirements

  • Please note that a Master's in Social Work degree is required for this position. (If you have not obtained this degree, please apply to one of out Outreach Worker II/Case Manager II positions)
  • Must be eligible to drive a company vehicle to transport clients. For insurance purposes, drivers must be at least 25 years of age, have a valid CA Drivers License, and must pass a Motor Vehicle Report

  • This position may involve variable scheduling including early mornings, late evenings, and weekends. For example, there may be an early morning shift once per week, or a weekly evening outreach. Must be available for this type of variable schedule
  • Must be authorized to work in the United States for any employer

Nice To Have

  • Case management experience with City Net’s target populations (people affected by homelessness, substance use disorders, chronic health and mental health concerns, domestic and intimate partner violence, and/or human trafficking) strongly preferred
  • Spanish Fluency
  • Experience and contacts with homelessness services organizations in Orange County
