Position Title: Social Worker

Job-Type: Full-Time

Schedule: Monday-Friday, 8-hour shift

Qualifications

• Bachelor's Degree in Social Work, or 1-2 years social work experience. (Required)
• Previous experience in skilled nursing or long-term care facility. (Preferred)
• Master's degree in Social Work (Preferred)

Essential Duties:

Provides direct and regular services including:

• Resident and family counseling
• Crisis intervention and community referral.
• Program development and implementation
• Participation in interdisciplinary team conference.
• Maintenance of documentation as required by state and federal regulations.
• Utilization of community resources.

Participates in the following meetings:

• Serve as advisor to the Resident Council for assigned area.
• New Family Orientation.
• New Resident orientation
• Family Council.
• Networking and educational meetings.

Additional Responsibilities:

• Responsible for intra-facility transfers with the Admission Coordinator.
• Responsible for developing awareness of community resources.
• Involved with in-service education in the department, facility, and the community.
• Utilizes the services of a MSW Consultant through private consultation and educational opportunities.
• Advocates for and protect the rights of residents.
• Prepares patient histories, service plans, and reports.
• Participates in planning for improving health services by interpreting social factors pertinent to development of program.
• Provides general direction and supervision to workers engaged in clinic home service program activities.
