Position Title: Medical Social Worker

Supervisor: Reports to Executive Director

Position Summary: Provides direct Social Work services for the underserved in the Lewis-Clark Valley and rural surrounding areas. This person will conduct needs assessments for individuals and make appropriate referrals to community organizations, assist volunteers in conducting needs assessments and making referrals, and conduct follow-up on insurance applications. The position requires assistance with data collection, possible grant writing to secure additional funding, and other duties as assigned.

Responsibilities:

• Works as part of a multidisciplinary team.
• Maintains patient, personnel and clinic confidentiality.
• Conduct social work needs assessments and make appropriate referrals to social service organizations for the uninsured and underserved in the region.
• Provides assistance and oversight to volunteers conducting needs assessment and assists with referrals as needed.
• Confirms adequate follow-up with all patients applying for insurance to assure a successful experience.
• Conducts follow-up for social service referrals.
• Documents appropriately in patient chart
• Assists with data collection to ensure reports are accurate.
• Works collaboratively with medical, lay staff, and volunteers to ensure care continuity.
• Assists with yearly strategic planning as needed.
• Assists with grant writing as needed.
• Other duties as assigned.

Qualifications: Master’s degree in social work with an Idaho license is preferred. Bachelor’s degree with equivalent experience will be considered. Candidate must possess experience working in a medical setting and possess knowledge of community resources.

Experience in conducting group sessions preferred. Candidate must work well in a multi-disciplinary free clinic team-oriented setting and understand the target population’s needs.

Job Type: Part-time

Pay: From $22.00 per hour
