The Health Center at Franklin Park, an American Baptist Homes of the Midwest community, is looking for a knowledgeable and compassionate Licensed Practical or Registered MDS Nurse to join our team.

This is a Part-Time day shift position 2 days a week

As a LPN or RN MDS Nurse, you'll perform a critical role in collaborating and overseeing the full collaborative interdisciplinary assessment and care planning process for 64 residents, provide Minimum Data Set (MDS) and Resident Assessment Instrument (RAI) requirements, and encourage each resident's ability to maintain or attain the highest physical, mental, and psychological well-being, through community programs and procedures in compliance with applicable laws and regulations, ABHM mission and core values.

What you will need:

• Graduate from an accredited school of nursing; current Licensed Practical Nurse or Registered Nurse license in Colorado or current compact state license.
• Long Term Care and/or Skilled Nursing experience is helpful, but not required
• If RN, RAC-CT Certification helpful or willing to obtain within 60 days from hire
• Knowledge and experience with RAI process required
• Covid-19 vaccination is required to work in a healthcare setting
