We are seeking an outstanding LVN to join our Recuperative Care team as LVN Medical Coordinator at our Glendale Facility. As our Medical Coordinator you would bring a compassionate, team player and “whatever it takes” attitude to the team. You will provide the highest quality care and service to our guests, being able to communicate effectively an clearly to them as they prepare to transition out to independent care. Pertinent Information
The schedule for this position is Sunday - Thursday from 9:00 AM - 5:30PM. This is not a traveling position. The position is on-site at our Glendale recuperative care facility (36-bed program).
The salary for this role is $60k/year. In addition to the annual salary, NHF currently has an Inflation Pay policy, providing additional pay through the end of July 2023. NHF reserves the right to stop the policy at any time.
In compliance with National Health Foundation's Covid-19 policy, all employees must be fully vaccinated against Covid-19, including their booster shot when eligible, with the exception of medical and/or religious exemptions.
What Does Success Look Like As A LVN Medical Coordinator At NHF?

  • Assist in facilitating hospital discharge process and patient assessment/engagement into our Glendale Recuperative Care program.
  • Complete a medical assessment intake and discharge plan of care for our guests, maintaining current client charts on existing online database up to date and in compliance.
  • Conduct medical/wellness checks and education on medications, preventative measures, overall general health, and medical insurance for our guests are they getting ready to transition out of our facility.
  • Coordinate and link participants to follow-up medical appointments and medical home.
  • Participate in case management team meetings and monthly staff meeting, tracking participant information on existing database specific to progress and plan of care.
  • Perform administrative tasks such as charting, patient summaries, and answering phones.

Requirements
What Does A Candidate Bring To The Organization?

  • Current LVN License in good standing
  • Must have up-to-date TB test and immunization record.
  • Must have active BLS certification.
  • Work experience with homeless patients would be preferred but not required. Experience working with substance abuse and dual diagnosed clients.
  • Proven ability to stay calm and focused when working under stressful situations.
  • Experience or the openness of working with patients from diverse socio-economic, ethnic, and cultural backgrounds.
  • Effective communication and social skills are essential as you build and maintain relationships with clients and fellow employees.
  • Proven ability to meet challenging deadlines, prioritize. and organize effectively.
  • Computer proficiency in MS Word, Excel, Outlook, web browser and proficient typing skills.
  • Bilingual in another language commonly spoken in Los Angeles County (preferred).
  • Be open to new experiences, can do attitude and willing to help others.
  • The duties as described will required frequent standing and walking. The employee will frequently bend/stoop, squat, reach above the shoulders, twist and turn, kneel, and push/pull. Must utilize conversational speech for effective verbal communication. Noise level is high in some client care areas.
