Job: LPN

Location: Elmwood Park, NJ

Pay: $25-35/Hr

Duration: 6 Month with possible extend

Shift: Day (9 am-5:30pm Monday through Friday)

Required:

6+ Month recent Experience

LPN License

BLS

Covid Vaccinated

Job Description:

· Performs gap closure activity as outlined, and escalates issues as needed

· Evaluates customers’ needs and can advocates on patient’s behalf to connect them to resources, services, and potential solutions related to their health condition

· Provides patients with education on the proper use and setup of diagnostic device (ie. Blood pressure machine, blood glucose monitors, pulse oximeter, etc).

· Effectively communicates with agency staff, liaisons, other Care Facility staff, patients, and family/caregivers

· Manages Time effectively to deliver accurate and appropriate patient care

· Maintains Confidentiality of patient and proprietary information

· Maintains a professional image

· Participates and attends required in services, trainings and weekly meetings

· Build and maintain a working relationship with the front end and pharmacy team to enhance patient engagement and experience

· Ensure all provided marketing, informational, and educational materials are available

· Ability to navigate technology to document interactions following standard procedure for facility Operations

· Assisting patients in aisle by providing them education on OTC and first aid products, escorting patient to pharmacy for patient specific questions

· Upkeep functionality of technology to ensure ability to properly capture service

Salary: $25.00 - $35.00 per hour
