Maxim Healthcare is looking fora Licensed Practical Nurse (LPN) to assume responsibility and accountabilityfor the application of the nursing process and the delivery of patient care,under the supervision of a Registered Nurse (RN). The LPN will consistentlyperform according to nursing standards and is accountable in managing patientcare and assisting others in the management of patient care. The LPN providesservice in accordance with Maxim’s philosophy, standards and policies.

Essential Duties and Responsibilities:

• Utilizes thenursing process to plan, implement, and evaluate patient/client care and/orservices provided to client(s) receiving contracted services, as required
• On eachassignment, provides skilled nursing care/services in accordance withprescribed orders
• Identifies anddocuments changes in patient/client physiological and psycho-social condition
• NotifiesDirector/Supervisor, and contracted customer designee if applicable, when:
• Physiciannotification is required in order to obtain necessary orders forintervention(s), per state regulations
• Changes in patient/client condition have occurred
• Identifies and prioritizes actions based uponpatient/client care requirements and/or contracted customer requirements
• Ensures that documentation reflects patient/clientcollaboration including participation in plan of care creation and maintenance,physician correspondence/collaboration, expected patient/client outcomes,nursing interventions employed, and other items as required by contractedcustomer
• Performs/provides interventions, treatments, andmedication administration, according to identified priorities, the plan ofcare, facility policies and patient/client care outcome standards as applicable
• Monitors and documents effectiveness of interventions,treatments, and medications
• Assures patient/client safety at all times

• Monitors the safety and effectiveness of thecare/services environment and equipment used, including universal precautions
• Executes consistent, effective interpersonalcommunications and maintains positive, professional relationships withpatient/client and contracted customer stakeholders

• State and Local Laws

• Policies and procedures of contracted customers

• Attends case conferences, team meetings and educationin-services, as required
• Performs other duties as assigned/necessary

Minimum Requirements:

• Graduate of anaccredited practical nurse or vocational nursing program preferred
• Currentlylicensed as a Licensed Practical Nurse or Licensed Vocational Nurse license inspecific state required
• Complies withaccepted professional standards and practice required
• One year priorprofessional nursing experience preferred
• Willingness to participatein skills advancement training or novice nurse program if applicable
• Current CPR, asapplicable, for direct patient care
• PPD or chestx-ray if applicable
• Successfulcompletion of Maxim Orientation and Compliance Training, and all other assignedtrainings as applicable required
• Successfulcompletion of Maxim’s Competency Program required
• Current HealthCertificate (per facility contract or state regulation) required
• Must meet allfederal, state, local, and contract requirements
