JOB DESCRIPTION

JOB TITLE: Life Enhancement Coordinator EFFECTIVE DATE: April 2023

DEPARTMENT: Life Enhancement CLASSIFICATION: Non-exempt

REPORTS TO: Admissions Director SUPERVISES: Activity aides

Mennonite Friendship Communities is sponsored by congregations from the Anabaptist/Mennonite tradition of faith in the greater South Hutchinson area. ROLE: Supervise, plan, organize and promote the daily neighborhood programs to meet the individual psychosocial, spiritual, and physical needs and desires of the residents in cooperation with families and other departments.

GENERAL DUTIES & RESPONSIBILITIES:

Supervisory

  • Assists with development and review of department policies and procedures.
  • Assists with development of budget.
  • Assists with supervisory functions for the department including interviewing, hiring decisions, scheduling, evaluations, disciplinary actions and approving attendance and requests for leave.
  • Work closely with other departments, keeping communication open.

Life Enhancement Coordinator Functions

  • Greet and welcome new residents and their families to their new neighborhood.
  • Evaluate psychosocial needs and activity preferences.
  • Develop activities that are specific to each individual resident’s wishes, desires and enjoyment.
  • Assist with discharge planning as needed.
  • Assist with room changes as requested.
  • Assist residents with a Monthly Resident Council meeting.
  • Escort and drive to activities outside the facility.
  • Assist and enlist the assistance of others in wheeling and walking residents to activities as needed.
  • Work with and/or supervise volunteers as directed.
  • Assist in other neighborhoods when needed
  • Place and remove wander guards. Check wander guards Monday through Friday and upload into the computer. Maintain Elopement list, send updates through email, and add chart flag.
  • Distribute and prepare chronicles and newspapers daily.
  • Daily update meal menu on three boards.
  • As needed, provide cover for Transportation including maintaining appointment schedule/calendar, drive residence to appointments, or provide an escort to residence.
  • Decorate monthly according to theme. Update bulletin boards monthly.
  • Coordinate last respect walk.

Recordkeeping/Communication

  • Maintain open communication with residents and families. Listen to concerns and questions. Assist them in finding answers to their questions. Facilitate communication with other departments as needed.
  • Visit with residents on an individual basis and in small and large groups
  • Develop close relationships with residents in neighborhood.
  • Document in resident charts on admission, quarterly and more frequently when needed.
  • Complete Mini Mental and Depression assessments as required.
  • Complete Activity and Social Service sections of the MDS.
  • Develop a Care Plan to address needs. Attend Care Plan meetings.
  • Develop and distribute Monthly Activity calendars for neighborhood with the assistance of the residents and in coordination with other Neighborhoods.
  • Write and distribute Monthly Newsletters for the neighborhood.
  • Complete daily documentation of activities
  • Maintain updated pen pal, Veterans, newspaper, library book lists and inform corresponding partner with updates.
  • Keep open communication with
  • Purchase needed activity items for the neighborhood, staying within a fixed budget. Work in cooperation with other neighborhoods in the purchasing of these items.
  • Leave facility rooms and equipment in the same or better condition than found, when activities are completed.
  • Recognizes abnormal signs and symptoms which signify a physical change in resident’s condition that would require nursing intervention. Reports change to Charge Nurse immediately.

Other Duties

  • Flexibility with schedule to work day/evening/weekends.
  • Perform other duties as assigned.

  • Assists in developing creative and innovative ideas to increase efficiency, effectiveness and increased customer satisfaction.

QUALIFICATIONS:

Experience: Six months of related experience is preferred.

Education: Kansas Certified Nurses Aide. CDL preferred, but not necessary. Completion of the Activity

Director/Social Service Designee course for Kansas

Skills: Enthusiasm, courtesy, and interpersonal skills.

Ability to work well with others to achieve desired outcomes.

Excellent verbal and writing skills.

Computer literate with mastery of Microsoft Word and ability to learn other varied programs.

PHYSICAL DEMANDS:

Requires frequent walking, standing, sitting, and bending, as well as frequently lifting up to 30 pounds.
