Landmark Home Health is looking for Licensed Practical Nurses in Monroeville, Penn Hills, Verona, Turtle Creek, McKeesport and surrounding areas with strong clinical skills, computer skills, and of course Home Health Care experience is not required but a PLUS!
Benefits:

Duties and Responsibilities:
- Provides nursing care following the established Plan of Care.
- Reports clinical findings to primary nurse on a daily basis.
- Evaluates the patient's progress.
- Informs the physician and other responsible personnel of significant changes in the patient's condition.
- Discusses with patient/caregiver any changes in the patient's care plan.
- Provides instruction to patient/caregiver regarding disease process, treatments and goals.
- Completes patient documentation within the established time frame assuring information is appropriate.
- Participates in interdisciplinary case conference when necessary.
- Assists the primary nurse in updating the nursing care plan at regular intervals and as necessary.
Qualifications:
- Current Pennsylvania Nursing License is required
- Minimum of one (1) year of current nursing experience preferred
- Valid current driver's license and auto insurance coverage is required
Working Conditions:
- Normal working hours are Monday through Friday, 8:00 am - 4:30 pm and one weekend per month or as needed
