Looking for a motivated, engaged , compassionate, licensed Physical Therapy Assistant for PRN work within the Ontario, Chino hills area.

This independent contract position works directly with individuals in their congregate living homes. Under the guidance of the lead physical therapist, the licensed physical therapy assistant will provide patient care focused on functional status, activities of daily living, independent living skills, fine motor coordination, home safety recommendations, adaptive equipment training, strengthening and maintaining general strength and mobility. The physical therapy assistant will ensure that the treatment plan is followed, performing interventions aimed at improving and enhancing the patient's wellbeing, and monitoring the patient’s progress. The successful candidate will also collaborate with supported living center staff ( nurse, direct support professionals, house administrator) to enhance the physical, mental, and social well-being for patients.

This position allows for flexible hours, efficient documentation, setting your own schedule, stability of hours, and working as a part of a caring team to provide compassionate care to individuals that will benefit from your care!
