This position will combine and utilize professional skills and expertise in the area of case management of patient care in the outpatient clinic providing support for the physicians, other therapists and staff to be able to serve their patients in an efficient manner. This position will be responsible for handling crisis that arise for patients daily and interfacing between the patients, families and providers within the department.

MINIMUM QUALIFICATIONS :

EDUCATION, CERTIFICATION, AND/OR LICENSURE:

1. Bachelor’s Degree in Social Work or related field required.

EXPERIENCE:

1. Three (3) years of experience in Social Work required.

PREFERRED QUALIFICATIONS :

EDUCATION, CERTIFICATION, AND/OR LICENSURE:

1. Current West Virginia Social Work licensure preferred.

CORE DUTIES AND RESPONSIBILITIES: The statements described here are intended to describe the general nature of work being performed by people assigned to this position. 1. Facilitates referrals of patients to providers.

2. Provide day-to-day oversight of case management staff assignments and tasks

3. Provide support and training/education to peer recovery coaches.

4. Takes crisis calls and works outcomes for patients.

5. Sends refill requests to providers and call pharmacy per direction of MD/NP/PA as needed.

6. Maintain a working knowledge of relevant medical/legal issues that impact on patient care, e.g., advance directives, child and elder abuse.

7. Provide education to patients and families around issues related to adaptation to the patient's diagnosis, illness, treatment and/or life situation.

8. Participate in multi-disciplinary health care teams.

9. Maintain working knowledge of and liaison with community agencies and resources.

10. Arrange, procure, and coordinate patient/family pre and post hospital needs.

11. Knowledge of hospital, medical center and/or health system resources to access and provide for patient care needs.

12. Document assessment, plan, interactions, and interventions according to departmental, hospital and/or health system guidelines and standards.

13. Assisting in obtaining prior authorizations from third party carriers.

14. Working with the CRC Inpatient units in securing beds for admission.

15. Preparing or assisting in the preparation of discharge letters.

16. Maintaining and updating group listings.

17. Scheduling of MRI’s, PET’s, etc.

18. Works with discharge planners and consult team to secure appointments.

19. Schedules new patient visits per established protocols.

20. Meets with patients face to face as needed to resolve crisis situations.

21. Assists Residents/front desk staff with resolving scheduling problems.

22. Covers in-basket messages for providers that are out of the office.
