Licensed Clinical Social Worker:St. Thomas Community Health Center, an FQHC, is building an integrated behavioral health program to meet the needs of patients with mild to moderate behavioral health issues in the context of a busy primary care and specialty care practice of 25,000 patients. We are looking for experienced LCSWs who have worked in medical clinics and are interested in developing and promoting this model of care. Our vision is to align LCSWs with the primary care providers to provide behavioral health assessments, short term counseling, crisis intervention and facilitate Psychiatric consultation with Psychiatrists/APRNs to further meet our goal of behavioral health integration in primary care. Through a hybrid approach of telemedicine and in person assessments and interventions the LCSWs role can offer workplace flexibility while improving behavioral health access for our patients.
