Position Summary
Provide individually tailored, strength-based, and collaborative case planning services to high risk families referred through Erie County Department of Social Services

Primary Responsibilities
  • Work intensely with referral sources, community service providers, and all family members to ensure collaborative efforts toward established service plan. Build upon support networks for each family and ensure adequate and stable supports are in place to stabilize families
  • Maintain intensive in-home contact, service provision, and therapeutic rapport with families which includes face-to-face counseling, active treatment, and intervention for a minimum of 10-15 hours per week per family
  • Engage families in a face-to-face in-home contact within 24 hours of referral. Assess, develop, and document in Connections an initial service plan with the family within 48 hours of acceptance of referral. Participate consistently in teamwork and collaborate with referral sources (e.g. ECDSS), court, community providers, and applicable agency staff
  • Ensure accurate, thorough, and timely completion and filing of progress notes, court reports, health/safety assessments, billing reports, correspondence, and other case-specific documentation. Prepare and document in Connections daily detailed progress notes highlighting the families identified goals and outcomes related to safety, permanency, and well-being. Maintain current ETO data
  • Utilize a Solution-Focused Trauma-Informed Care Approach to collaborate with ECDSS, family/natural supports, service providers and system partners (i.e. academic, medical, legal, spiritual, therapeutic providers, etc.) to identify precipitating safety & risk issues, family strengths/needs and establish a comprehensive service plan to achieve treatment outcomes
  • Develop, teach, model and evaluate effective techniques and interventions with parents/caretakers (i.e. parenting skills, behavioral management, and discipline) and children/youth (attendance, academics, anger management, behavioral modification and Solution-Focused Trauma-Informed Care)
  • Ensure all court mandated and/or recommended services are offered, utilized, and maintained via referral/linkage or provided directly to minimize risk, while enhancing the family’s safety, permanency, and overall well-being
  • Actively participate in required/offered training opportunities. Provide mentoring opportunities for student interns and volunteers within the agency as needed
  • Other duties as assigned
Required Qualifications
  • Bachelor’s Degree in Human Services related field with 4 years’ experience providing supportive services to children and their families
  • Current NYS Driver’s License and personal vehicle
  • Ability to build rapport with youth based on healthy boundaries, effective communication and mutual respect
  • Ability to exercise discretion and judgement in decision making
  • Ability to demonstrate sensitivity, compassion, patience, and active listening
  • Ability to effectively work with individuals from a diverse array of cultural backgrounds, beliefs, customs, and lifestyles
Preferred Qualifications
  • Master’s degree in Human Services related field with 2 years’ experience providing supportive services to children and their families
