Job Description

The basic functions of the Intake Therapist is to provide assessment, treatment, psychotherapy, case management and referral to broad range of clients. This may include both severely mentally ill clients as well as those dealing with adjustment issues. Clients can range in age from young children to the elderly. Support and education are provided for the significant others/collaterals of the clients as well. Assessments are provided in accordance with best practice and professional ethics and boundaries. In addition, the Intake Therapist will provide case management services and crisis intervention as needed. The Intake Therapist provides initial assessments, including telehealth assessments, which include but are not limited to comprehensive assessment, risk assessments, substance abuse screening, safety planning, and preliminary treatment.

Responsibilities:

  • Conducts initial intakes and completes clinical documentation pertaining to such.
  • Meets with prospective clients and asks a series of questions concerning social history, treatment and other pertinent information in order to compile data for the purpose of identifying level of care deemed medically necessary based on patient’s clinical presentation, determining provisional diagnosis categorized under ICD-10 diagnostic codes.
  • Provides clinical management of individuals and/or families in crisis and facilitates psychiatric hospitalization when necessary.
  • Provide therapy as needed to an ethnically and culturally diverse population of children, adolescents, and adults and complete required documentation for services provided.
  • Participates with all pertinent staff in the development, review, and delivery of treatment and advocacy.
  • Adheres to all current federal & state laws, as well as the agency’s policies and procedures, including the safeguarding of confidential healthcare information and compliance with the Health Insurance Portability & Accountability Act (HIPAA)
  • Reports immediately any suspected incidents of child abuse or neglect to the State Central Registry Hotline.
  • Demonstrates an awareness, appreciation and respect for diverse cultures and individual differences of clients and fellow employees.
  • Attend intake team meetings and related trainings.
  • Perform other related duties as assigned by Supervisor.

Qualifications

  • Master's Degree from an accredited school in Social Work or Mental Health Counseling
  • Must be licensed by the state of New York in professional discipline (LMSW, LCSW or LMHC)
  • Must be fluent in English and Spanish
  • Outstanding engagement, interpersonal communication skills, and ability to work collaboratively with other disciplines, including clinical and clerical staff.
  • Excellent time management skills and ability to multi-task is required.
  • Strong demonstration of professionalism.
  • Takes initiative, prioritizes, and adjusts to change well.
  • Excellent verbal and written communication skills that are timely and effective.
  • Proficient in Microsoft Office, including Outlook, Word, and Excel.
  • Able to work well in a fast-paced environment.
  • Maintains a positive attitude and is friendly, upbeat, and has a great “can do” personality.

Pay: $85,000.00 - $95,000.00 per year
