Overview:
As an Intake Coordinator,you will be responsible for receiving all incoming customer service calls for homecare/hospice services from members, patients, physicians and other referral sources. This includes acceptance of referrals and scheduling of appropriate services. Responsible for recognizing both expressed and implied customer needs and recommending the solution that will meet consumer s desired outcomes. Gather all information needed for verifying insurance eligibility, patient needs, and make the business decision regarding a referral to Home Healthcare (HHC) . Refers patients, members and providers to Presbyterian Home Healthcare(PHH) RN Home Care or Hospice Coordinator (HCC) staff when assistance is needed with a more complex array of services or to determine the appropriate levels of care required. Assisting patients and members in finding appropriate providers for Homecare/Hospice services. Serves as a resource person concerning home healthcare services availability within New Mexico and state border areas. Generates and trends referral, admission and scheduling reports as assigned.
Location & Commitments:
Two or more years experience working in a healthcare facility or organization (preferred). Customer service/ scheduling in a medical setting.

Ideal Candidate: Bilingual (Spanish/English) is a plus.
Location & Commitments:
Full-time - Non-Exempt.
Job is based on our PMG - Presbyterian Pan American
Work hours: Monday-Friday (40hrs per week) 8am-5pm -
Hybrid position: Open to remote: 3 days from home & 2 days in office.
Qualifications:
Two or more years experience working in a healthcare facility or organization (preferred). Customer service/ scheduling in a medical setting.
Education Essential:
High School Diploma or GED
High School Diploma or GED
- Meets with patients and families as appropriate. Receives incoming calls from multiple sources including members, patients and providers and transcribes the necessary information.
- Meets internal and external customer service expectations by recognizing both expressed and implied customer needs and recommending the solution that will meet the callers desired outcomes.
- Develops and maintains a working knowledge of all Home Healthcare/Hospice service and resources available within the community, state and state border areas.
- Enters the necessary demographic and clinical information in the PHH computer systems to process the referral and authorization of the patient and member into the computer system.
- Demonstrates knowledge and understanding of productivity goals that support the financial goals of the organization.
- Insurance and benefit verification completed and entered into PHH computer as assigned.
- Performs other duties and responsibilities as assigned
