Purpose:

Directs individuals interested in mental health treatment to the services which meet their needs. In a culturally sensitive and appropriate manner, provides and coordinates these services to program within the agency. Serves as advocate for C4’s mission, vision and brand.


Duties and Responsibilities:

The Intake Case Manager works with the other members of the Intake team to ensure that clients who are waiting for services with C4 will receive regular outreach. The Case Manager will assist in engagement with clients and will provide services prior to being assigned to a therapist for ongoing treatment. The Intake Case Manager will work with treatment teams to facilitate a smooth transition from waiting for services to working with a therapist. The Intake Case Manager will also work with staff on treatment teams to provide some services for clients who have been assigned to therapists. The Intake Case Manager will, as a part of the Intake team, manage referrals for mental health services and complete any paperwork as required, including timely entering of notes into C4’s Electronic Medical Record. The Intake Case Manager will have the responsibility to communicate with clients their current status of likely wait time to be linked to a therapist and also will communicate with other Intake staff what services are being provided to clients prior to being assigned to a staff therapist. The Intake Case Manager will consult with clinicians about clients as necessary in order to provide excellent services to clients and will also attend regular supervision with designated program supervisor and attend team meetings when deemed necessary.

Minimum Qualifications:

Bachelor’s degree in social work, psychology or a related field.

Clinical experience working with individuals

Well organized with a client-centered approach

Excellent knowledge of MS Office Suite

Previous knowledge of working with an EMR preferable
