Job description
Our Sacramento Home Health team is looking for an RN to become our Clinical Manager. As the Clinical Manager, you will effectively manage clinical operations, services, personnel performance, and office management on an ongoing basis. This is a crucial role to the success of our clinicians and ability to provide excellent patient care!

Pay begins at $125,000.00/year exempt. Negotiable, depending on experience. This role is onsite, M-F. 8:00am-5:00pm without exception. This role requires frequent overtime, weekends and holidays.

Responsibilities

    • Ensure that the client’s plan of care is individualized and appropriate, executed as written and reassessed by the appropriate health care professional when there is a significant health status change in the client’s condition, at the physician’s request and after hospital discharge. Ensure appropriate documentation is completed for all patients transferred to an inpatient facility.
    • Hold weekly Case Conferences to discuss client issues. Determine if clients that are planned for discharge meet the requirements for Management and Evaluation or any other skilled service.
    • Develop corrective plans congruent with the SOP and Policies and Procedures. Route the corrective plan to the Administrator and Vice President.
    • Recruiting, hiring, orientating and retaining of field and office staff in order to cover referrals and patient visit needs. Conduct general and job-specific orientation.
    • Supervise the day-to-day office and clinical operations.

Qualifications

    • Graduate of an accredited school of nursing. Bachelor's degree preferred, with a minimum of 3+ years of home health care experience.
    • Must have a minimum of 2+ years of nursing experience within the last 36 months.
    • Medicare home health experience required.
    • Current licensure in State, CPR certification.
    • Must have a good understanding of the Federal, State and local laws/regulatory guidelines governing the operation of a Home Health Mission Healthcare.
    • Have valid driver’s license with no restrictions and auto insurance meeting state’s insurance coverage minimums.
    • Supervisory experience not required but preferred.
