SUMMARY:
TCA Health, Inc. seeks a resourceful, compassionate candidate to provide outreach and linkage-to-care services for our patients and community members. The Care Coordinator will assist individuals living with and at-risk for HIV/AIDS in accessing and adhering to health care services and linking them with core clinical, PrEP (Pre-Exposure Prophylaxis) and supportive services. The Case Manager will provide a range of client-centered, confidential services including completing intakes, conducting assessments, providing referrals and conducting HIV/STI education to individuals at-risk and those diagnosed with HIV/AIDS. The position demands a high level of cultural competency in the unique needs of communities of color, particularly those in Hispanic and African American communities, and among the LGBTQIA+ community.
ESSENTIAL DUTIES AND RESPONSIBILITIES:
  • Manage an assigned caseload of clients.
  • Partners with clinical and supportive services team members to ensure the highest quality of client care is provided.
  • Implement PrEP (Pre-Exposure Prophylaxis) education and case management.
  • Provide ongoing follow-up to ensure PrEP adherence.
  • Meet with patients weekly to assist them in filling out PrEP medication coverage applications.
  • Collaborate with all individuals involved in PrEP care and program delivery for the purposes of supporting high quality PrEP programming.
  • Review and maintain up-to-date and accurate knowledge of emerging scientific research regarding biomedical HIV/STI prevention strategies, including PrEP, PEP (Post Exposure Prophylaxis) and related interventions.
  • Accurately complete all data collection forms
  • Conducts initial intake and ongoing assessments, medication and treatment adherence, and complete service plans.
  • Monitors progress toward individual client goals.
  • Assist with the development of comprehensive care plans with clients that integrates medication and treatment adherence into services.
  • Provides and tracks referrals and linkages to services.
  • Maintains client file, documents all activity, and maintains appropriate documentation of eligibility.
  • Conduct outreach and engagement at community events in collaboration with community partners and TCA Health mobile van.
  • Inputs all data into the client level database and/or electronic health record (EHR).
  • Submits monthly/quarterly/annual reports on program/client activities in a timely fashion.
  • Attend and participate in weekly, monthly, and quarterly meetings and trainings as required and assigned.
  • Performs other duties as required or assigned.
EDUCATION AND EXPERIENCE:
Education:
  • Bachelor’s/Assosciate’s Degree with a major in social work, health sciences, public health, sociology, or related subject.
Experience:
  • At least 1-2 years of experience working in community health, HIV/STI prevention, and/or the LGBTQ community is preferred.
Demonstrated Competencies:
  • Proficient in English.
  • Excellent communication skills, especially written and public speaking.
  • Able to manage time and set priorities.
  • Strong organizational skills. Proficient in Microsoft Office suites and internet.
  • Must be highly motivated, self-initiated, and flexible; prepared to work evenings and weekends as needed.
  • Previous case management positions are beneficial.

  • Able to communicate and respond effectively.
  • Will need to frequently crouch, kneel, stoop or bend.
  • Will need to move or lift up to 50 pounds.
  • Requires visual acuity to perform tasks requiring close vision and ability to adjust focus.
  • Required to be able to travel to meetings outside the organization’s facility, including some out of town travel.
WORKING CONDITIONS
  • Frequently works outdoors.
  • Normal office and health center environment.
  • Occasional evening and weekend work.
  • Will require working some evenings and weekends as it relates to instructing and supervising fitness classes and other programming.
