Essential Functions of the position are detailed below, and include any physical requirements below that.
1. Works independently and consistently exercises discretion and sound judgment in providing individual and group psychosocial counseling sessions. Provides assistance to clients who need help accessing and adhering to health care services and who require a high frequency of contact. Provides supportive medical case management services.
2. Conducts initial intakes and ongoing re-certifications in addition to maintaining appropriate documentation of eligibility for funding bodies.
3. Develops individual care plans which are clear and concise and assists clients in meeting their goals. Revises the service plan with the client every six months during the recertification. Ensures timely and accurate submission of care plans as required in all mandatory data systems.
4. Researches new community resources for unmet needs of clients; tracks and documents referrals, collaborates with other treatment providers and attends all client staffing's.
5. Records the distribution of bus cards. Ensures documentation of all resources provided to clients.
6. Maintains compliance with Medical Case Management and training requirements.
7. Organizes and facilitates Client Advisory Boards. Participates in community meetings and represents the agency regarding services provided as well as providing accurate and timely reporting to the department director of the meeting discussions.

Kneel and move from sitting, bending, kneeling or standing multiple times a day.

X Additional Requirements: Prefer Spanish speaking abilities. Completion of Medical Case Management Training within first 6 months after hire. On-going requirement is 12 hours of annual training in Medical Case Management.

Education and Experience Requirements:
Relevant Education:
Preferred: Master's Degree in Social Work, Counseling or Psychology
Minimum: Master's Degree in social services field

Relevant Experience:
Preferred: 3 years relevant experience
Minimum: 1 year social services experience

Certification/Licensure:
Preferred: LCSW or LCPC, Medical Case Management Training
Minimum: N/A
