Our Health and Social Care Coordinator Team currently consists of 5 members and we are looking for someone new to join our team who shares our values and patient centred approach.

Our team deal with incoming referrals across the Canterbury locality and surrounding coastal areas. We are responsible for responsible for assessing basic equipment needs, signposting patients to the most appropriate services when they have more complex needs and keeping the inter-professional team updated.

This is a role with joint accountability to Kent Community Health NHS Foundation Trust and Kent County Council, and you will need to demonstrate flexibility and adaptability to working in a dynamic environment. You will need to be an expert communicator across services and a leader in information sharing.

In this role you will be responsible for ensuring that patients are referred on to the most appropriate health, social and/or voluntary organisations in a timely manner. A telephone call or face to face assessment in the patient’s home environment may be required to ascertain the needs of the patient.

You will provide a single point of contact for GP’s and assist them in improving coordination of care. You will coordinate and attend inter-professional meetings, providing appropriate feedback and develop effective working relationships with integrated short term and practice linked teams, GP practices and other agencies to ensure that our patients receive a consistent, integrated response.

Additional Working Pattern: 7.5 hours each day Monday to Friday
