Mennonite Village is a not-for-profit, equal opportunity employer, CCRC (Continuing Care Retirement Community) facility that provides 24 hours/7 days a week care located in Albany Oregon. We are currently hiring a Full Time Health Services Coordinator-LPN for Lydia s House Memory Loss Unit designed to care for elderly persons who have Alzheimer s disease or related dementia. Our LPN will combine compassion, patience and nursing skills working in conjunction with the entire team to provide the best possible care for the residents. Must be a certified LPN in the state of Oregon; have computer experience and knowledge of Senior care. Sign On Bonus - Payable $500 at 90 days worked and $500 at 180 days worked. Shift will be 2PM-10:30PM Sunday- Thursday.

ESSENTIAL FUNCTIONS:

  • Performs initial nursing evaluation of newly admitted residents; establishes service plans to meet residents needs. Ensures that physician s orders are received and followed.
  • Regularly evaluates health related needs of residents; participates in service plan meetings and in-services; modifies nursing care to meet current resident s needs. Performs focused assessments within license scope.
  • Assists Staffing Coordinator in providing training and guidance to resident assistants and Team Leaders.
  • Supervises Team Leaders (TL) (including training, assigning work, evaluating performance, counseling, and administering discipline. Conducts audits of care and services to verify that resident service plans are being followed and accurately reflect each resident s needs.
  • Follows through on verbal medication change orders received from resident s physician.
  • Ensures appropriate documentation of all nursing services provided.
  • Initiates appropriate preventative and rehabilitative nursing procedures.
  • Acts as leader of Care Team and point of contact for all health-related issues.
  • Works closely with Administrator and RN keeping them informed of resident service issues and problems.
  • Interacts with families, pharmacists, physician office staff, and others in order to ensure that a resident s needs are met; promotes good facility public relations during interactions.
