Under the general supervision of the Vice President of Clinical Operations, the Health Home Director is responsible for the planning, growth, overall administration and oversight of all Health Home activities, including the Behavioral Health Home, the Opioid Health Home and the demonstration of the Certified Community Behavioral Health Home. This includes supervision of staff and the development and implementation of program policies and procedures.


PRINCIPLE DUTIES AND RESPONSIBILITIES:

  • Oversees the overall design and ongoing implementation and oversight of the Behavioral Health Home, the Opioid Health Home and the Certified Community Behavioral Health Clinic.
  • Ensures enrollment into the Health Home on behalf of providers
  • Serves as a liaison between the health homes partners and MDHHS staff/contractors.
  • Ensures Health Home staff coordinates with Health Home Partners to identify a beneficiary’s optimal setting of care.
  • Coordinates and utilizes Health Information Technology (HIT) with the Health Home Plan team to maximize care coordination and care management.
  • Champions practice transformation based on health home principles.
  • Collects and reports on data that permits an evaluation of increased coordination of care and chronic disease management.
  • Monitors Health Home performance and leads quality improvement efforts.
  • Designs and develops prevention and wellness initiatives and referral tracking.
  • Serves as DWIHN’s liaison to MDHHS Behavioral Health Home and Certified Community Behavioral Health Clinic (Integrated Healthcare) Divisions.
  • Provides oversight to all consultation, training, general supervision and oversight as well as technical assistance across systems for Health Home initiatives.
  • Convenes collaborative workgroups to service implementation issues and/or barriers.
  • Analyzes the service delivery system and makes recommendations to DWIHN regarding service needs.
  • Reviews funding needs in support of services in consultation with the Vice-President of Finance and makes recommendations for Board Actions.
  • Ensures compliance with the Health Home and CCBHC certification process and certification requirements.
  • Manages the evaluation and selection of providers who meet the certification standards for Health Homes.
  • Establishes and monitors the infrastructure to support Health Home initiatives in care coordination and providing required services.
  • Works with local and regional Universities and their staff to support Research and Evaluation activities aligned with Health Home Initiatives.
  • Creates presentations for state and national meetings related to Health Home initiatives.
  • Provides training and technical assistance.
  • Represents DWIHN at other forums as assigned by the Vice-President of Clinical Operations or their designee.
  • Works collaboratively with Directors of the units within DWIHN to meet the overall goals and objectives of DWIHN.
  • Supervises program and support staff as assigned.
  • Develops and monitors productivity and efficiency standards.
  • Performs additional related duties as assigned.

REQUIRED EDUCATION:

A Master’s Degree from a recognized college or university in the Human Services, the Social Services, Nursing (a Bachelor’s Degree will be accepted), Public Health, Public Administration, Healthcare Administration, Health Management, Psychology, Counseling, Social Work or a related field.


REQUIRED EXPERIENCE:

Five (5) years of full-time paid professional post Bachelor’s degree experience working in a human service, social service, mental health or behavioral health setting.

AND

Three (3) years of full-time paid professional experience working in Community Mental Health programs and working with agencies that provide services for persons with behavioral health disorders.

AND

One (1) year of full-time paid professional experience working with Behavioral Health Homes or closely related experience.


REQUIRED LICENSE(S)

A valid State of Michigan Driver’s License with a safe and acceptable driving record.


WORKING CONDITIONS
:

Work is usually performed in an office setting but may require the employee to drive to different sites throughout Wayne County and the State of Michigan. This position can work remotely with supervisory approval. Currently this position is primarily a remote position.
