GENERAL DESCRIPTION:
Under general supervision of the Program Director, provides case management of assigned caseload working with assigned Agency consumers, their families, and the foster families, gathering all required documentation, providing instructing counseling activities, assisting with treatment goals, and placement planning and care, in compliance with the requirements of applicable local, state, and federal regulatory and/or other accrediting agencies.

REPORTING RELATIONSHIPS:
This position reports directly to the Foster Care Supervisor.

RESPONSIBILITIES (illustrative and not intended as inclusive; specific responsibilities may vary):

(1.) Coordinates or provides timely services and casework needs to all assigned Agency consumers, families, and foster families as required by Program-specific procedures, applicable contract terms, and/or applicable local, state, and federal regulatory and/or other accrediting agencies.

(2.) Ensures accurate, professional, and timely completion of all required reports, logs, note, and other paperwork as required.

(3.) Monitors and provides necessary services to Agency consumers, families, and foster families.

(4.) Ensures accurate, professional, and timely completion of all Agency-required reports, paperwork, and filings.

(5.) Attends, conducts, or participates in all case conferences, hearings, assessments, home visits, etc. as required.

(6.) Coordinates treatment services with appropriate local, state, or federal agencies and/or community resources.

(7.) Immediately apprises the appropriate Agency personnel of any potential emergency situation (supervisor and/or Program Director).

(8.) Provides other Agency-related duties as assigned.

(5.) Complies with the Agency’s Employment Policies & Procedures and Standards of Conduct.

(6.) Maintains as confidential all matters related to Agency affairs and refrains from internal and external discussions related to any employee, consumer, financial, or administrative issue except as required as part of job performance.

Requirements:
QUALIFICATIONS (required at time of offer):
(1.) High school diploma or GED.

(2.) Prior experience working with consumers in a social-services setting preferred.

(3.) Excellent written and verbal communication/presentation skills.

(4.) Excellent organizational, analytical, and interpersonal skills.

(5.) Reliable transportation.

(6.) Able to work a flexible schedule, including weekends and holidys, with “regular “business hours set by the Agency.

(7.) Able to perform on-call services 24-hours per day, seven days per week to assist Agency consumers and/or provide crisis intervention as required.

(8.) Prior experience with Microsoft Office and related applications or other IBM-compatible word processing program.

(9.) Satisfaction of all employment eligibility and Agency hiring requirements.
