FAMILY SUPPORT PARTNER FOR HOME FAMILY VISITING PROGRAM IN PHILADELPHIA

The Family Support Program is a new, evidence-based prevention program that has shown incredible outcomes for families receiving these services that will now be offered in Philadelphia through Gemma Services. The Family Support Program is a home-visiting model that will be offered to families with young children in the Philadelphia area seeking assistance. The Family Support Partners will be working directly with these families in their homes and communities by providing the Family Check-up Model and Everyday Parenting curriculum. There will be five (5) Family Support Partners delivering services in this program.

Job Specifics: The Family Support Partner will provide the short-term interventions offered by the Family Check-Up and Everyday Parenting models to those seeking assistance. The Family Support Partners will also help connect families with needed resources and referrals. These services occur directly in the home with the family, so it is important for the Family Support Partners to truly partner with the families and approach their work with a collaborative approach and respect. The Family Support Partners conducts their work with the support of an internal team to help with intake, reporting and building community connections. The caseload size for a Family Support Partner will range from 20-21 open cases, serving each family for an average of 8 weeks.

This is a Monday-Friday, full-time position that is based out of our Preheim Center in Southwest Philadelphia but also has meeting space in Mt. Airy. The position works mostly in the field throughout the city of Philadelphia and must be able (and interested!) to travel daily and independently.

This position requires a high school diploma or the equivalent. A Bachelors degree in social work or a closely related field is preferred. Experience in providing services to children and their families is required, with home visiting or early intervention experience preferred.
