Job Summary: Provides AHS system-wide care coordination, navigation and advocacy for patients seeking abortion and other family planning services, including outreach and engagement, development of the care plan, scheduling of the medical treatment sought, and linkage of the patient with resources (transport, financial, community-based services). Serves as liaison to referring providers (internal and external to AHS) for Family Planning services by managing referrals and provides administrative support to the Director of Family Planning.


DUTIES & ESSENTIAL JOB FUNCTIONS
: NOTE: The following are the duties performed by employees in this classification, however, employees may perform other related duties at an equivalent level. Not all duties listed are necessarily performed by each individual in the classification.


1. Serves as primary liaison to patient from initial request for pregnancy termination to post-abortion care or other Family Planning services from start to completion of care.

2. In case of abortion care, explains abortion options and abortion process to patient for full scope of abortion services (medication and procedural treatments in first and second trimester of pregnancy). For complex contraception referrals, explains clinical care process.

3. In collaboration with the care team, schedules initial consultation, preoperative visit (if applicable), and treatment such as surgical procedure in the Procedure Clinic, Operating Room or induction at Family Birth Center and post-abortion visit.

4. Assists with coordination of care in conjunction with other services relevant to patient’s social needs, such as arranging rides, providing appointment reminders, obtaining medical records, communicating with specialty clinics and services, and consulting care teams.

5. Tracks patients receiving first trimester medication abortion to confirm they attended follow-up visit, perform follow-up calls to assess ongoing medical or social needs.

6. Maintains waitlist for patients seeking procedural care and schedules patients from the waitlist.

7. Provides preoperative patient education and performs post-procedure calls to assess ongoing medical or social needs. Routes ongoing medical concerns to provider in a timely manner and assists with social needs directly or through collaboration Social work or case management.

8. Manages Epic internal/external referrals queue for Family Planning services.

9. Answers direct line for community referrals such as Genetics counselors, Maternal Fetal Medicine, Radiology and other outside providers and coordinate patient’s care under supervision of Director of Family Planning.

10. Explains scope of services and scheduling procedures to referring providers.

11. Works with ambulatory nurses to help with abortion and pregnancy loss scheduling and care coordination, as needed.

12. Ensures follow-up care or communication, if needed following care completion at AHS with referring providers.

13. Performs warm hand-off directly or work with social work team for patients who need additional services beyond Family Planning.

14. Maintains updated summary of Family Planning services yearly and as needed.

15. Participates in outreach efforts (site visits and other forms of communication) with community organizations, including CHCN partners in the East Bay.

16. Update referring organizations and internet databases yearly or when major service changes arise (abortionfinder.org, 211, Access Reproductive Justice, etc.).

17. Assists in managing dashboard for family planning related quality metrics based on Slicer Dicer reports.

18. Performs chart reviews for non-report generated data for quality tracking.

19. Performs monthly data entry of abortion volume for #Recount Society of Family Planning national research study and National Abortion Federation (NAF) institutional membership data requirement (all de-identified aggregate data on volume, safety and security metrics).

20. Provide operational support (scheduling, technology setup, troubleshooting) for trainings and workshops.

21. Facilitates staff accessing National Abortion Federation trainings, including security and clinical in-services

22. Maintain records for staff who have completed Values Clarification and Attitude Transformation (VCAT) workshops and Policies & Procedures competency trainings.


Qualifications


Preferred Experience: One year of above experience working in direct-patient care that provided abortion services and/or other gynecologic procedures; working in a community outreach program performing duties such as intake, peer counseling and screenings, referral management and patient scheduling.


Preferred Licenses/Certifications: Community Health Worker (CHW) Certification.


Preferred Languages: Fluent in English and Spanish, spoken and written.


Required Education: Bachelor’s degree in a related field (health, social science or a related field such as psychology or counseling).


Required Experience: Three years of work experience in a community-based health care service capacity, or the equivalent of two years of relevant full-time experience working in a community outreach program performing similar duties.


Required Licenses/Certifications: Valid California Driver’s License.
