Responsible for job:
  • Coaching, teaching, assisting and monitoring individuals with disabilities, in the performance of their employment duties.
  • Assist in the implementation of goals and attainment of objectives by developing and implementing intervention techniques to help participants/workers develop necessary work skills, achieve job stabilization and rehabilitation.
  • Responsible for providing necessary support to workers on and/or off their work site to ensure they access and/or maintain employment. Driving as needed.
  • Prepares and submits written reports to funding agencies.

The Job Coach/Vocational Counselor is required to:
  • Assist/empower individuals to identify and achieve recovery goals of employment, education and community integration. Meet with clients in the field for updates, intake or interview sessions.
  • Develop and implement vocational recovery plans for individuals diagnosed with mental health conditions.
  • Work within ACCES VR guidelines and adhere to terms of that contract.
  • Report monthly progress utilizing state education department forms and submit all required billing via zip file system. Complete quality indicator report as needed/requested by ACCES VR.
  • Provide and initiate job development, job placement, job coaching, assessments, work skills training, job preparedness and community outreach as needed.
  • Maintain a caseload of individuals for the purpose of assessment, rehabilitation planning and goal setting for employment or education. Complete documentation and follow up for assigned caseload.
  • Collaborate with ACCES VR counselors regarding clients’ goals, placements, assessments and employment.
  • Initiate all referrals received though ACCES VR for case opening.
  • Receive supervision by and report to the Program Director/Manager.
  • Function as a vital part of an interdisciplinary team environment across several agency departments and programs.
  • Complete and submit billing to funding sources monthly. Generates and maintains various computer-generated reports for the purpose of tracking information and analyzing outcomes; maintains accurate program data for the purpose of generating and submitting written reports in a timely fashion; prepares and delivers written and verbal reports.
  • Ensures program documentation remains in compliance with all regulatory standards, i.e.: VESID, OMH, DMH and Agency.
  • Meets regularly with supervisor to discuss issues and/ or concerns and brainstorms suggestions and resolutions.
  • Attend applicable meetings, seminars and trainings geared towards securing job slots for Vocational services participants. Complete required trainings to maintain CRS contract guidelines.
  • Establishes and maintains good agency image and customer relationships; including visiting job sites and providing assistance to employers and / or participants as needed.

Qualifications:
  • Associates’ or Bachelor’s Degree preferred with experience in rehabilitation placement, job coaching, or related fields, and / or enrollment in a specialized and related course approved by the agency.
  • Valid New York State driver’s license and access to a vehicle required.
