JOB TITLE: Employment Services Coordinator

RATIONALE: The Employment Services Coordinator is responsible for the implementation of individualized employment skills training and for assisting individuals assigned to the program to obtain and/or maintain their optimal vocational level. The Employment Services Coordinator organizes and completes job development and job search activities with consumers, job coaching at job placement sites, provides vocational counseling services, and provides linkage with other services providers. Serves as HRC 753 Safety Officer and is a member of the Referral and Behavior Management Committees. Chairs the work services weekly meeting.

BASIC DUTIES:

1. Acts as liaison with the Division of Rehabilitation Services(DRS) counselor for persons enrolled in employment skills services by performing tasks such as developing individualized vocational placement plans, scheduling, staffing’s, completing DRS progress reports and keeping the DRS counselor informed regarding the status of individual cases.

2. Assists work supervisor(s) and job coaches to establish and maintain a work-like atmosphere in all agency worksites by performing tasks such as assisting with direct supervision, community employment standards and making recommendations to the Laundry Manager and/or Janitorial Supervisor

3. Serves as Case Manager for Pre-Employment individuals by performing tasks such as maintaining documentation of disability, assigning to a Work Services work site for on-the –job-training, ensuring job accommodations and restructuring as necessary, providing job readiness training, monitoring performance ratings according to quantity and quality standards to meet criteria and to provide linkage, referral and follow-up for needed services including DRS.

4. Maintains and implements a written program of Community Based Employment Services that meet accrediting body standards

5. Completes monthly DRS Billing Reports

6. Coordinates and implements community outreach activities to promote all aspects of Employment Services including pre-employment participants, competitive employment participants and develops working relationship with potential employers.

7. Coordinate accident and incident follow-up, as needed.

8. Complete 753 safety inspections as assigned

9. Ensure annual drills are completed, documented and are compliant with Commission on Accreditation of Rehabilitation Facilities (CARF) and other governing bodies.

10. Attends and participates in all assigned committee meetings.

11. Provides 1:1 job coaching for Employment Services customers working competitively in the community, as needed.

DESIRABLE QUALIFICATIONS:

1. Bachelor’s degree in human service field or equivalent life experiences.

2. Excellent communication skills with employers and customers. .

3. Ability to work independently as well as part of a team.

4. Willingness to learn and utilize an Electronic Healthcare Record system (Qualifacts: Carelogic)

5. Ability to relate positively to other professionals, family members, and the community.

6. Computer skills computers including but not limited to MS outlook, MS Word, MS excel,

printers, copiers, internet based time and attendance systems, Outlook 365, and accessing

network ‘shared drives.’

7. Currently possess the Illinois Direct Support Person (DSP) credential

MINIMUM QUALIFICATIONS:

1. At least 18 years of age.

2. Ability to pass basic First Aid and CPR certification training.

3. Ability to obtain IL DHS Direct Support Person (DSP) credential (education provided by agency)

4. Ability to pass pre-employment background checks, fingerprinting, and drug testing

5. Ability and willingness to comply with agency procedures and state regulations

6. Possess a valid HS Diploma or GED from a nationally accredited high school

7. Ability to work independently and part of a team

8. Possess time management skills

PHYSICAL REQUIREMENTS (to be met with or without reasonable accommodations):

JOB HAZARDS:

1. Blood Borne Pathogens: This job may involve exposure to blood borne pathogens. Universal precautions are required.

2. Ergonomics: This job may involve repetitive motion and/or keyboard related activities. Ergonomic procedures are required.

3. Environmental: This job involves exposure to noise and extreme seasonal heat.

4. Chemical Exposure: this job may involve of hazardous chemicals. Safer handling procedures are required
