DAY-TO-DAY

This full-time job coach position with our human services nonprofit works the day shift during business hours.

As an Employment Case Manager in human services, you are responsible for establishing trust with our program participants by serving as a job coach. You conduct comprehensive intakes with our participants to understand their needs, strengths, interests, aptitudes, experiences, and capabilities. In collaboration with participants, you help develop a service plan known as a Family Contract Amendment (FCA) that outlines their short- and long-term employment goals. You meet with each participant at least once a month to provide case management services, such as documenting their progress toward their goals and ensuring they're clear on program requirements.

In addition, you build effective partnerships with external providers to make sure participants have access to the services and programs they need. You maintain exceptional record-keeping as well as make requests for support services in accordance with our policies to help participants successfully accomplish their FCA. If a participant misses an appointment, you reach out to them and help them reschedule. Using your strong customer service skills, you also maintain communication with the medical and behavioral providers of your participants to access important information.

At times, you run reports through our FedcapCARES program and use that data to plan daily and weekly tasks. You sometimes work with a targeted caseload that focuses on a specific group, such as two-parent families or individuals enrolled in higher education. REQUIREMENTS FOR AN EMPLOYMENT CASE MANAGER

  • Bachelor's degree OR equivalent experience
  • Demonstrated track record for providing exceptional customer service
  • Valid driver's license
