Position: Early Intervention Service Coordinator

Department: Early Intervention

Reports to: Early Intervention Director

Shift: Daytime; occasional evenings and weekends

Hours: 15hrs/week with possible full-time work (37.5hrs/week)

Job summary

Under the direction of the Program Director, provides developmental screening, assessment, direct service, and guidance to families of children birth to three years of age who are at risk for, or experiencing developmental delays; uses skills in an area of specialization: nursing, occupational, physical, or speech therapy, psychology, or social work.

Essential Responsibilities

· Functions as a multi-disciplinary team member in the screening and developmental assessment of children birth to three years of age who are at risk for or experiencing developmental delays; provides direct services, including “hands-on” treatment, in home and/or center-based settings according to the needs of the family and child; assists in service planning for all program participants.

· Assesses the family’s needs and explains the Individual Family Service Plan (IFSP) process, in collaboration with the family and in accord with their stated needs and the developmental needs of the child, develops an IFSP within 45 days of referral to the program; monitors the provision of all services identified in the IFSP; informs the family about and assists in developing a transition plan when children are preparing to leave the Early Intervention Program.

· Provides service coordination, supportive family counseling, family support services including interagency coordination, helping families identify other service needs and assessing them (e.g.
SSI, financial, equipment, medical, et c.): with parental permission, collaborates with other agencies and medical and social service providers to support the child and the family; empowers families to advocate for their rights and needs.

· Provides education and guidance to families regarding individual children’s growth in perceptual/fine motor, gross motor, cognition, language and communication, self-care, and personal/social domains, provides information on parenting issues and community services, well baby care, nutrition and home management of specific health problems.

· Assists with planning and leading or co-leading center-based groups for children and/or parents, as assigned: plans and carries out activities for the group to address IFSP goals and objectives for each group participants.

· Attends regularly scheduled staff meetings and participates in training and ongoing staff development and supervision.

· Documents interaction with children and families including the date, duration and content of contact and the discipline of the provider; prepares reports in a timely manner and maintains case records including IFSP updates, assessment reports, transition plans and other forms in accordance with MDPH operational standards and agency policy.

· Shares expertise in own specialty through formal and informal teaching of team members and other staff.

· Maintains the required productivity requirement of the program.

· Performs other duties as required or assigned.

Additional Responsibilities

· Attends all staff meetings and trainings unless excused by supervisor

· Uses all means of agency communication including email, mail (mailbox), and phone (voicemail) to remain well informed of agency activities and client needs

· Maintains confidentiality according to agency policy

· Meets with supervisor regularly

Minimum requirements

Minimum educational requirements, in accordance with Massachusetts Department of Public Health operational standards, are as follows:

Mental Health:

A minimum of a master’s degree from an accredited recognized educational
institution in a. counseling psychology, clinical psychology, developmental psychology, or educational psychology

OR
b. Current licensure as a Licensed Mental Health Counselor (LMHC) or as a Licensed Marriage and Family Therapist (LMFT) by the Massachusetts Board of Allied Mental Health and Human Services Professionals
OR
c. A minimum of a Master’s degree from an accredited recognized educational institution in Mental Health Counseling, Marriage and Family Therapy, or a Related Field and “license eligible” for LMFT or LMHC with official written verification by the applicant’s accredited educational institution.

· One year of experience in the field of infancy/early childhood.

· Massachusetts Driver’s license and car.

· Current certification in infant/child CPR and First Aid.
