We are seeking a full time Data Manager to support our Manhattan Health Home site.

SUMMARY OF POSITION:

The HH Data Manager provides data entry oversight of the Health Home Care Management Program. This position is responsible for analyzing data gathered and assists with maintaining all internal and external data management systems. The position also is responsible for monitoring data errors within the Care Management Program and implementing corrective action plan. The HH Data Manager oversees the work done by the data specialists. The Data Manager develops and implements reports that improve the overall operations of the Care Management program.

ESSENTIAL FUNCTIONS:

· Oversees the collection, monitors/maintenance of internal tracking logs, agency reports, and error reports

· Oversees monthly data reconciliation; data analysis and reporting

· Ensures data entry into multiple clinical databases, including TREAT, Excel, eClinicalWorks, AIDS Institute Reporting System, and other Health Homes systems is performed by data specialists staff

· Monitors, reviews, and ensures reports to the leads are accurate

· Ensures data specialists monitors and reports any inactive MAs

· Provides EHR support and training

· Responsible for the oversight and accuracy of all case openings, case closures, bottom ups and top down assignments, and submitting weekly data report to Vice President

· Compiles information for special reports and maintains contact with other departments on related projects

· Works collaboratively on quality assurance / quality improvement activities

· Maintenance of data-related records

· Responding to standard requests for information regarding data

· Compiling information for special reports and maintains communication with other departments on related projects

EDUCATION/EXPERIENCE:

Bachelor’s Degree with 1-2 years relevant experience

Salary Range: $44,790.20-$51,606.10/ yearly
