The role of Crisis Diversion Care Coordinator requires a Bachelor’s/Master’s degree in a behavioral health related field or a Bachelor’s/Master’s degree in Education with at least 9 hours of credit in a behavioral health related field. Minimum of one year work experience with children in the mental health field or related social service field. Requires the willingness and ability to work irregular hours as needed to ensure quality standards are met in service delivery.

Will work as a part of the Crisis Diversion Team to provide immediate intervention for individuals admitted to CRS CCBHC who are at immediate risk of admission to local hospital emergency departments, psychiatric hospital inpatient units, and jails. Provide immediate crisis interventions whether on phone or in person; including a threat of suicide or harm to others; utilizing skills/interventions to de-escalate individuals who are agitated, anxious, angry, or in other vulnerable emotional states. During daytime working hours (while main office is open), the crisis diversion CM will be responsible for answering Daytime crisis phone and responding to crisis situations within the office (i.e. med clinic, lobby, etc…). Collaboratively develop emergency safety plan/safety promise with individual and/or family that ensures safety, matches the individual’s competency and skill level, and incorporates the person’s strengths and supports. Responsible for follow up on clients who screen into suicide protocol after initial evaluation appointment. This will consist of a 48 hour follow up contact with the client and the 2-week reassessment of PHQ-9, Columbia, and safety plan. Participate in verbal de-escalation and interventions in emergent situations and be willing to assist other team members as needed to maintain a safe and secure environment. Will be available to assist any walk-in with a crisis screening and in the moment safety plan to reduce risk of hospitalization. This may look like exploration of coping skills, social supports, education on triggers and mental illness/substance abuse (expansion of crisis walk-in hours to all day during business hours). Responsible for hospital/general PCP referrals faxed into the agency for psychiatric/psychological issues. Responsible for calling client and screening for services as well as setting appointment for walk-in or initial evaluation with intake specialist. Partners with each client to provide information about service options and the process of care. Overnight/weekendduties will include answering the afterhours crisis calls via phone or iPad. This will include completion of crisis diversion form and all required documentation within EHR. Will also follow up with the individual in crisis within 24 hours of call. Will staff crisis cases with supervisor and appropriate team upon office opening next business day. CDC 21 information completed. Prepare and send letters for follow up care including, but not limited to, missed appointments, expired plans, and discharged clients.
