POSITION: CPS Program Specialist/Activities Coordinator
STATUS: Full Time; 37.5 hours / week
SCHEDULE: Daylight hours; Monday through Friday
LOCATION: New Horizon Senior Center - Penn Hills

Milestone Centers, a subsidiary of Partners For Quality Inc., is a nonprofit, community-based human services agency supporting individuals with behavioral health challenges and intellectual & developmental disabilities in communities throughout Western Pennsylvania. Our Agency oversees 20 residential settings, two outpatient offices, three adult training facilities, a prevocational enterprise program, a consumer drop-in center, habilitation services, and two Health Care Quality Units in Butler and Warren, which train professionals in 18 counties. Milestone Centers has earned certification from the Sanctuary Institute for providing trauma-informed care.


To oversee and manage the programming for assigned caseload of individuals in the Intellectual and Developmental Disabilities department and maintain compliance with all applicable ODP and Dept. of Aging regulations. Duties include, but are not limited to:


ESSENTIAL FUNCTIONS AND CORE RESPONSIBILITIES

1) Demonstrates and reflects a commitment to the mission and values of Milestone by serving others with respect, dignity, and compassion

2) Utilizes Sanctuary Model’s shared values, the Seven Commitments and tools to conduct daily tasks

3) Coordinates, completes, and distributes all assessments as required by licensing chapter regulations as well as assessments based on health, safety, risk reduction, and client satisfaction initiatives

4) Participates in the individual plan process including development, team reviews and implementation, in accordance with applicable licensing chapter

5) Reviews individual plan updates, critical revisions, and annual reviews for accuracy and communicates needed corrections and revisions to the Supports Coordinator. Maintains documentation of correspondence with the Supports Coordinator and plan/support team. Manages, coordinates and provides staff training regarding the needs of the individuals as identified in the plan

6) Provides, coordinates and oversees activities for the individuals in accordance with the individual plans

7) Supports the integration of individuals in the community

8) Supports the communication needs of the individuals and promotes involvement with families and friends

9) Reviews Daily/Service Notes for completion and accuracy and incorporates data collected into Progress Notes


DEPARTMENT SPECIFIC FUNCTIONS AND RESPONSIBILITIES

1) Fulfill the requirements of the Activities Coordinator per Department of Aging, Chapter 11 regulations

2) Develop and provide meaningful, quality activities that meet the Core Services requirements for Therapeutic Activities.

3) Design and plan activities that meet the functional needs of the clients, this is to include Individualized activities, small and large group activities, active and spectator participation, intergenerational experiences, involvement in community activities and events, outdoor activities as appropriate and opportunities to volunteer.

4) Assist with adapting activities in order to meet the functioning level and interests of all participants.

5) Develop and post a monthly activity’s calendar that shows a full schedule of activities throughout the programming day.

6) Provide direct supervision of clients at required ratio during scheduled work hours, including but not limited to: personal care including lifts and transfers, transportation, implementation of SEEPs, Behavior Plans, Restrictive Procedure Plans, and Individual Support Plans, in addition to mitigating and addressing day-to-day crisis management

7) Addresses any unmet safety or welfare needs of the clients apparent during program hours

8) Implements and documents goal plans, community participation notes and other required documentation

9) Assists supervisor with the purchasing and maintenance of program supplies and vehicle needs.

10) Relevant and reasonable work assigned by Program Supervisor, Manager, or Director

QUALIFICATIONS

  • Master’s Degree or above from an accredited college or university and 1 year of work experience working directly with persons with disabilities OR
  • Bachelor’s Degree from an accredited college or university and 2 years of work experience working directly with persons with disabilities OR
  • Associate’s Degree or 60 credit hours from an accredited college or university and 4 years of work experience working directly with persons with disabilities
  • Ability to lift and transfer without restriction
  • Act 33 & Act 34 clearances
  • Valid PA Driver’s license, if transporting
  • Office of Developmental Program CPS certificate within first 60 days
