The Counselor (Outreach - Polk County) will provide clinical evaluations, ongoing treatment services and case management for patients who are referred for therapy/medication management services.

• Conduct patient assessments and evaluations; develop and implement treatment plans along with transition planning for patients in treatment as well as in Polk and Hillsborough County Jail.
• Provide case management services for patients.
• Provide individual, group and family therapy, as necessary within treatment, Polk and Hillsborough County Jail settings.
• Prepare and maintain clinical records for all assigned patients.
• Track and submit reports as required timely.
• Participate in clinical consultations and team meetings internally and with external teams as scheduled.
• Make referrals to appropriate resource agencies for all patients.
• Provide feedback and collaborate with stakeholders and community resources.
• Provide crisis intervention, when needed.
• Participate in staff development activities.
• Ability to travel to Polk and Hillsborough County Jail and other community sites to complete evaluations, assist with treatment engagement and outreach services.
• Other duties as assigned.

Education and Experience:

• Master's degree with minimum of 2 years of experience in a similar role (required).
• Valid Driver's License.
• Must be able to pass background and drug screen prior to start date.

Knowledge, Skills & Abilities:

• Ability to read and interpret documents such as safety rules, operating instructions, and procedure manuals. Ability to write routine reports. Ability to speak effectively. Ability to solve practical problems and deal with a variety of concrete variables in situations where only limited standardization exists. Ability to interpret a variety of instructions furnished in written, oral, or schedule form. Knowledge of 12-step program, behavior modification, harm reduction, case management and addiction.

Job Type: Full-time (Non-Exempt).

Compensation: $22.59 USD per hour ($47k/yr).
