The Community Health Worker will provide patient education, patient outreach and resource navigation in alignment with population health programming and initiatives. This may also include conducting health screenings and questionnaires allowable by licensing (medical assistants, certified nursing assistants, personal trainers, etc). Additionally, the position identifies community resources to improve or enhance the patient's quality of life and assists patients with referrals such as behavioral health, SNAP benefits, health insurance, WIC, and food resources.
The Community Health Worker will also act as an Affordable Care Act Application Certified Counselor that provide eligibility and enrollment assistance to uninsured patients of TCA Health Center and residents in our approved service area.
Role & Responsibilities:
  • Provide patient in reach and outreach, health education and resource navigation for individuals and groups; virtually and in-person.
  • Help clients in connecting with and utilizing resources, including scheduling appointments and follow-up care.
  • Coach patients in effective management of their chronic health conditions and self-care and assist patients in understanding care plans and instructions.
  • Work collaboratively and effectively within the team.
  • Actively participate in and assist with population health and wellness programming.
  • Effectively work with people (staff, clients, doctors, agencies, etc.) from diverse backgrounds.
  • Participate in community networks pertaining to health issue.
  • Must have ability to travel to and from health center and program sites on the far Southside of Chicago, including Riverdale, Roseland, Pullman, West Pullman, Chatham, Hegewisch, Dolton, Calumet City, Calumet Park, South Holland, and surrounding suburbs.
  • Develop strong working relationships with the appropriate organizations and partner organizations
  • Track and report, monthly, quarterly, and annually enrollment and assist data via shared drive
  • Work with the Patient Services Supervisor to identify existing patients that need access affordability options.
  • Conduct questionnaires with varying survey methods throughout the community
Education & Experience:
  • At least 1 year of experience in community health, public health, or social services setting strongly preferred.
  • Associate’s or bachelor’s degree in Health Education, Health Promotion, or other demonstrated experience/education.
  • Must be able to learn and use electronic medical record systems, and have intermediate mastery of Microsoft Word, Excel, and PowerPoint.
  • Bilingual is a plus.
  • Completion of Certified Assisters and Illinois Navigator certification process within 90 days of hire
  • Training and education in medical assisting (MA, RMA, CMA) or nurse assisting (CNA) is a plus.
  • Punctual, reliable, and willing to learn.

  • Ability to develop and deliver presentations.
  • Skill in the use of personal computers and related software applications.
  • Skill in organizing resources and establishing priorities.
  • Ability to design, prepare, deliver, and modify course curriculum and associated educational aids.
  • Ability to plan, assess, and evaluate programs.
  • Knowledge of community health services and agencies.
  • Ability to effectively interpret needs and act as an advocate for a specified client population.
  • Able to collect and accurately report data, document and input data as needed
  • Strong interpersonal and customer service skills
  • Ability to maintain professionalism in high pressure situations
