Your day.

As a Community Health Worker, you need to know how to:

• Collaborate with the multidisciplinary team to support the patient's plan of care, including but not limited to, promoting self-management activities, assuring access to services, providing individualized support and reinforcement of education, determining the need for continued services, and connecting patients and families with available community resources, as needed.
• Assist in ensuring the safe care of patients, including facilitating the appropriate exchange of information among all caregivers.
• Participate in patient appointments with providers, as needed.
• Serve as support for patient self-management. Interview patients and families to assess current strategies for self-care and available support systems to manage health needs.
• Build effective working relationships with patients, in order to assist navigation of the healthcare system.
• Work effectively with community partners to identify opportunities and to support patients' access to appropriate resources.

Your experience.

We hire people, not resumes. But we also expect excellence, which is why we require:

• Associate's degree.
• Current BLS certification endorsed by the American Heart Association.
• 2+ years of healthcare or social services experience in an acute, post-acute, or ambulatory care setting.

Preferred:

• Bachelor's degree.
• Demonstrated experience with various populations, including those with limited financial resources, limited education background, and communication barriers.
