We are looking for a full-time Community Health Worker for our Raymond location. Responsibilities will include
- cultural outreach to assist patients in understanding their plan of care
- coordinate patient health services
- educate patients on adopting positive health choices
- assist patients with follow up medical appointments.
This role has a large focus on conducting community outreach and health education to inform the community of the many services offered by our dynamic integrated health team.
Bachelor’s Degree is strongly preferred. Successful completion of a Community Health Worker training program is preferred. Must have reliable transportation.
