Job description

Full-time Community Health Worker. This position will be based in Kalkaska County and physically located within the Emergency Department at Kalkaska Memorial Health Center in partnership with KMHC.

Duties:

· Through home visits and in clinic and community settings, works one on one with clients to secure information such as medical, psychological, and social factors contributing to client’s situation.

· Records information in database and works to eliminate barriers to good health.

· Advocates for clients and acts as a liaison between clients and other service providers.

· Helps clients and families understand, accept and follow medical recommendations.

· Provides health care enrollment services, Medicaid Outreach and DHD#10 services outreach in community settings.

· Covers Kalkaska County.

• Position will be physically located within the Emergency Department at Kalkaska Memorial Health Center in partnership with KMHC.

Requirements:

· High School diploma or equivalent required.

· Strong people and counseling skills.

· Certified Community Health Worker required or ability to acquire.

· Ability to effectively interview clients in clinic, home, and community settings, present information one-on-one and/or small groups to clients and community organizations.

· Ability to work within a team and clinic environment.

· Ability to utilize a computer electronic medical record.

· Reliable transportation and occasionally work non-traditional hours.

· Experience in social work and/or public health preferred.

Pay: $17.59 - $19.94 per hour
