HRPP/NICP Community Nursing services are provided through the Arizona Department of Health Services High Risk Perinatal Program. The Community Health Nurse provides support for transitions from hospital to home; developmental coaching; referrals to community resources; interconception support and education to the family; as well as conducts developmental, physical, psychosocial, and environmental assessments, and a maternal well-being and post-partum screening.

General Duties and Responsibilities (With or Without Accommodations):

  • Uses independent judgment and discretion in setting daily priorities to complete tasks.
  • Demonstrates sensitivity to linguistic, cultural, social, economic, individual and role differences among persons and families served.
  • Identifies high risk infants and children through hospital and community referrals.
  • Explains the benefits and responsibilities of the program to families in a manner which is sensitive and responsive to individual family needs.
  • Works in collaboration with families to assess their educational, social, health and economic needs, develops an intervention plan and assists them in attaining their established goals.
  • Identifies and disseminates relevant resource and service information to families and co- workers as appropriate.
  • Maintains contact with families, primarily through home visits, to foster trust, promote communication to families and provide guidance toward meeting the goals of the family.
  • Functions as an effective team member with co-workers and the community to build productive, collaborative relationships and to achieve program goals and objectives.
  • Keeps appointments, is punctual, and utilizes time effectively.
  • Shares pertinent information with team members regarding work progress, successes, barriers, concerns that may have an impact on program quality.
  • Provides in-service training for hospital and community agencies upon request.
  • Participates in internal/external relevant committees and meetings, as requested.
  • Communicates through verbal and non-verbal means accurately, concisely and professionally.
  • Communicates in a courteous, positive and respectful manner.
  • Interacts with co-workers and the community to build productive, collaborative relationships to support the achievement of service goals.
  • Understands and maintains private and sensitive information within the confines of need-to- know parameters, using HIPAA guidelines and rules.
Requirements:

Education:

  • Arizona Licensed Registered Nurse, BSN or MS preferred

Experience:

  • Pediatric birth to five care; Newborn Intensive Care Unit

Regulatory:

  • Clean criminal background check and the ability to obtain and maintain a level one clearance card.
  • Certification in Adult/Child/Infant CPR. (If not current, employer provides)
  • Valid, unrestricted Arizona driver’s license and the willingness to use personal vehicle in the course of employment. Must meet minimum requirements of agency insurance carrier.
  • Possess an automobile in good working order, with required automobile registration and insurance, for use as needed in job responsibilities.
