Davis Behavioral Health, Inc. is seeking a full-time employee to perform duties as a Clubhouse Generalist (Case Manager/Social Service Worker) at Journey House, a program which promotes the Clubhouse Model of Psychiatric Rehabilitation to members with severe and prolonged mental illness. This position is based at the clubhouse located at 2250 North 1700 West in Layton, Utah. The successful candidate will report to the Journey House Program Director and join a strong team of dedicated professionals.

Journey House Clubhouse (JH) is a community based psycho-social rehabilitation program where adults with persistent mental illness can rebuild their self-respect and confidence. We provide a supportive and accepting community in which members can gain a sense of belonging and accomplishment through education, productive work, and meaningful relationships.

Journey House Clubhouse is not a treatment facility. It is a unique supportive and collaborative setting where those who participate in our programs are "members" rather than clients. JH is one of over 300 Clubhouse programs world-wide that is certified by Clubhouse International.

What is your role at Journey House?

Journey House is looking for an individual to for in a fast-paced environment that is flexible, adaptable, and capable of having fun!

The belief that recovery is possible for people with mental illness must be a core social work value as you will work with the members to identify their strengths and challenges and as they work towards the goal of pursuing meaning and productive work, personal, and social life. The members are the foundation of Journey House. You should have exceptional interpersonal skills and the ability to develop and maintain meaningful relationships with people from various walks of life and diverse backgrounds. You need to be a strong team member (willing to provide coverage in all units, as needed), self-starter, creative thinker, and can engage many individuals at one time.

What you will do on the job.

Daily responsibilities include, but are not limited to the following:

Clubhouse Management

  • Planning, coordinating, and executing daily unit work with members.
  • Working side by side with members to complete unit work and tasks.
  • Applying and modeling the Clubhouse Standards, in daily tasks.
  • Creatively encouraging and motivating members to participate in their recovery.
  • Organizing, executing, and participating in some evening, weekend, and holiday activities.
  • Actively participating in the fundraising efforts for Journey House.
  • Completing frequent outreach of members who are not in attendance for each unit.
  • Assisting to meet the community needs of members.

Documentation Requirements

  • Developing Personal & Individualized Treatment Plans with members.
  • Completing reviews of the treatment plans with members.
  • Completing bi-weekly comments on progress notes with member in unit.
  • Completing documentation in adherence to DBH standards.

Other Responsibilities

  • Providing supportive life coaching to members.
  • Providing on-site job coaching to members as needed to maintain successful Transitional Employment Placements (T.E.P.), Supported Employment, and Competitive Employment in the community. Participate in Transitional Employment as placement manager.
  • Participate in 2-week clubhouse training.
  • Adhere to Clubhouse International Standards and Philosophy
  • Other responsibilities as assigned by Program Director or Supervisor.

Schedule and Classification

This is a full-time, non-exempt position and is normally assigned to weekdays. However, DBH reserves the right to require overtime, weekend work, and/or assignment to evening or night shifts, depending upon operational need to assist with member activities. Some in and out of state travel may be required. All DBH employees may be re-assigned to another program depending on Center needs.

Compensation

The hourly rate for this position is outlined below

  • $17.00 per hour for candidates without a bachelors degree.
  • $18.00 per hour for candidates with a bachelors degree.
  • $21.00 per hour + for candidates licensed as a Social Service Worker (SSW) in the State of Utah.
    • 1% per year for each year of licensure between 0 and 10 years.
    • 1% per year for every three years of licensure 10 years and beyond.

Experience and Skills:

Candidates licensed as a Social Service Worker (SSW) or a Licensed Practical Nurse (LPN) in the State of Utah are preferred. Candidates who are not currently licensed but are actively pursuing SSW licensure or those that do not have a SSW license, but are eligible for Targeted Case Management Certification may also be considered.

Davis Behavioral Health utilizes an electronic medical record, which requires proficiency in computer-related skills.

Meeting monthly face-to-face service expectations and productivity standards is also required.

The selected candidate must successfully pass a pre-employment criminal background check and drug screening. DBH will not hire individuals currently using illegal drugs or abusing alcohol; therefore, making employment at DBH conditional upon successfully passing pre-employment drug screening by providing a valid sample. Candidates who are offered employment are required to report for a drug screen at an Intermountain WorkMed location within 24 hours of being requested to do so by the DBH Human Resources Department.

Davis Behavioral Health works with Federal grants and must comply with the Federal drug laws, including no use of medical marijuana.

Targeted Case Management Certification

Candidates who do not hold a professional license must complete certification as a Targeted Case Manager (TCM) within the first 30 days of employment.

Qualifications to become a certified Targeted Case Manager through the State of Utah Division of Substance Abuse and Mental Health include:

  • 400 hours of work, volunteer, and/or education experience in human services or a related field
  • Required 40-hour case management practicum and examination provided by DBH during the TCM certification class.

Preference will also be given to candidates who are bilingual English/Spanish.

The successful candidate must also have a clean driving record to operate agency vehicles and/or transport clients, when necessary. The ideal candidate will have experience working with clients that have sever mental illness, are hard working, can work both independently and successfully within a small outreach team.

EXPERIENCE REQUIRED

Prefer Clubhouse Model of Psychiatric Rehabilitation experience and/or willingness to work as Generalist Practitioner.

SKILLS/ABILITIES

Must be able to exercise judgment, work independently and effectively with a team under general direction.

PHYSICAL DEMANDS

The physical demands described here are representative of those that must be met by employee to successfully perform the essential functions of this job.

The employee must be able to drive and must have a valid driver’s license for six years minimum with clean driving record to be covered by DBH insurance. The employee is required to stand, walk, reach, carry, and kneel. The employee must be able to walk on uneven surfaces and climb stairs. The employee must occasionally move or lift 25-50 pounds. The employee must be able to sit at computer workstations for up to 3 hours a day. The employee must be able to perform multiple job tasks at places of Transitional Employment Sites, equivalent to the physical demands of working at the Clubhouse.
