Description and Responsibilities:

Open Sky’s Rapid Response HUB provides urgent and timely access to services for adult, children and youth who are seeking support with emotional, mental health and behavioral health challenges. Our services start in one place where individuals are coupled with a member of our team, who uses the “whole person” approach to address a variety of needs including behavioral health, substance use, navigation of the medical system, housing/food insecurities and benefit enrollment. The Rapid Response HUB is funded through a SAMHSA Certified Community Behavioral Health Clinic Grant and focuses on providing care to individuals in the traditionally underserved Latinx, LGBTQIA+ communities, Veterans, Justice-Involved and people experiencing homelessness.


The Clinical Intake Coordinator will assume lead responsibility of the intake workflow of our Rapid Response hub. They will ensure proper procedures are adhered to, so that all intake appointments are appropriately booked and authorized while providing initial insurance verification prior to the client visit. The Clinical Intake Coordinator will use brief assessments to evaluate the urgency of the presenting problem and assign individuals to the clinical team, as well as schedule and conduct in-person assessments and maintain contact on a regular basis according to contractual requirements and individual needs.


The schedule for this position is Monday-Friday days, with one late day.


Other Key Responsibilities:

  • Performing outreach and engaging with individuals within prescribed timelines.
  • Filling out client intake forms and assigning clients to the appropriate staff member/program.
  • Coordinate access to services/fulfillment of integrated health needs with providers including Behavioral Health, Substance Use Services, Primary Care Physician, and others as directed by the care plan.
  • Ensure safe transitions in care for members moving between care settings including identifying and arranging necessary residential placement/at home supports.
Qualifications:
  • Master’s degree in social service or related field, required.
  • Licensed or Licensed Eligible, preferred. (LICSW, LCSW, LMHC)
    Salary will reflect level of licensure obtained.
  • At least 1 year of training and/or experience in safety assessments, individual biopsychosocial assessments and intervention, psychoeducation and/or other support services that align with Open Sky’s clinical approach.
  • Experience with adults with mental health and substance use disorders preferred.
  • Excellent assessment, documentation, and verbal communication skills required.
  • Valid driver’s license and acceptable driving record.
