What You Will Own and Improve in the Role

• Completing detailed needs assessments focused on individuals with CVD and COPD
• Entering patients into the Partners Improving Patient Health Outcomes (PIPHO) program through nursing assessments
• Facilitating referrals to ancillary and community services
• Assisting care teams with development and patient rosters
• Overseeing and guiding CNAs, Community Health Workers, and Social Services who directly work with PIPHO

What You Will Bring to the Position

• Interpersonal skills to work alongside multiple patient and coworker personality types
• Ability to work well in a team environment, while having the self-starter motivation to work on your own
• Advocacy for quality patient care
• Understanding of social determinants to healthcare

To Be Successful

This position requires a current Washington State Nursing License (RN) and for you to have completed a 2 or 4 year nursing program from an accredited program. You will have prior experience working in a clinical or home setting. Show us your experience and any extra certifications you may have!
