Share Care USA is looking for someone who has the love for people to be a compassionate and diligent Client & DSW Service / Care Supervisor
Client & DSW Service / Care Supervisor:
- Creating and updating personalized care plans for clients.
- Collaborating with clients' families, friends, and social supports in developing treatment plans.
- Ensuring clients plan of care requirements are met.
- Instructing and educating DSW’s on the best and most productive way to provide services to clients that follow the plan of care
- Conducting regular follow-ups with clients & DSW’s to evaluate progress, promote continuity of care, and ensure healthy outcomes.
- Updating monthly client & DSW work calendars.
- Provide weekly / quarterly face-to-face visits with all clients as evidenced by signed face-to- face visit forms.
- Provide biweekly / monthly phone contacts with clients as evidenced by phone record logs.
- Will be on an On Call rotation. Must fill in as DSW if needed.
- Will need to live in region to accommodate emergency staffing requirement and rotating On Call.
- All other assigned tasks to benefit client, DSW and agency needs while following LA State Regulation.
- Identifying and recruiting high-quality healthcare employees.
- Will provide love, support and value to everyone along the way.
Job Type: Full-time
Education:
- Bachelor's Degree (Human Services Field)
Experience:
- Human Service- 1 year (Preferred)
License/Certification:
- Driver's License (Required)
- Willingness to travel

Pay: $12.00 - $17.00 per hour
